三孔法腹腔镜胆总管探查术的临床效果分析

摘　　要：目的探讨三孔法腹腔镜胆总管探查术(LCBDE)的临床应用价值。方法回顾性分析郑州大学第一附属医院肝胆胰外科2020-01—2020-12行LCBDE的91例胆总管结石患者的临床资料。按手术方法分为三孔法组(36例)和四孔法组(55例)。比较2组患者术前的一般资料、术中情况和术后临床指标。结果2组患者术前的一般资料差异无统计学意义(P>0.05)。除了三孔法组的手术时间长于四孔法组,差异有统计学意义(P<0.05)外,2组患者的其他术中情况和术后临床指标差异均无统计学意义(P>0.05)。结论除了手术时间较长外,三孔法和四孔法LCBDE的效果相仿。可在严格把握适应证的基础上应用。对于术野显露困难,操作复杂的患者,应选用四孔法实施,必要时应中转开腹。Objective To investigate the clinical value of three-port laparoscopic common bile duct exploration(LCBDE).Methods The clinical data of 91 patients with choledocholithiasis who underwent LCBDE in Department of Hepatobiliary and Pancreatic Surgery of the First Affiliated Hospital of Zhengzhou University from January 2020 to December 2020 were retrospectively analyzed.According to the operation methods,they were divided into three-port method group(36 cases)and four-port method group(55 cases).The preoperative general data,intraoperative conditions and postoperative clinical indicators of the two groups were compared.Results There was no significant difference in preoperative general data between the two groups(P>0.05).Except that the operation time of the three-port method group was longer than that of the four-port method group,and the difference was statistically significant(P<0.05),there was no significant difference in other intraoperative conditions and postoperative clinical indicators between the two groups(P>0.05).Conclusion Except for the longer operation time,three-port LCBDE has similar effects as four-port LCBDE and can be applied based on strictly the indications.For patients with difficulty in exposing the surgical field and complicated operations,four-port LCBDE should be selected for implementation,and if necessary,it should be converted to laparotomy.
