深圳西部社区2型糖尿病患者糖尿病足健康知识与行为掌握调查现状、危险因素分析

摘　　要：目的探讨深圳西部社区2型糖尿病患者糖尿病足健康知识与行为掌握调查现状、危险因素。方法选取2020年2月至2021年2月深圳西部社区中200例2型糖尿病患者,对患者的足部护理知识掌握现状、足部自我护理行为现状调查,并对影响因素进行分析,同时随机抽取患者进行分组,对照组患者接受常规护理,研究组患者接受“知信行”模式护理,分析2组患者的护理结果。结果社区200例患者评估中,足部护理知识评分(20.35±2.11)分,为中等水平,标准分为71.20分,足部自我护理行为评分(18.92±1.57)分,结果较差,标准分为58.21分;单因素分析中,影响患者足部护理知识、足部自我护理行为的因素包括文化程度、病程时间、是否接受过健康指导、以及是否定期复查(P<0.05);对社区患者足部护理知识、足部自我护理行为的影响因素分别进行多元逐步线性回归分析,自变量为文化程度、病程时间、是否接受过健康指导、以及是否定期复查(P<0.05);Pearson相关性分析中,患者的足部护理知识评分,与足部自我护理行为呈正相关(r=0.271,P=0.000);研究组护理干预后的足部护理知识掌握评分(50.32±2.51)分、以及足部自我护理行为评分(30.12±2.06)分,相较于对照组患者的足部护理知识掌握评分(43.25±1.25)分以及足部自我护理行为评分(22.81±1.87)分明显较优(t=25.214、6.274,P<0.05)。结论目前患者的足部护理知识掌握度、以及足部自我护理行为现状较差,主要受到文化程度、病程时间、是否接受过健康指导、以及是否定期复查因素的影响,且两者之间存在正相关;对患者采取“知信行”模式护理干预,可有效改善患者情况。Objective To investigate the diabetic foot health knowledge and behavior as well as mastering current situation and risk factors in patients with type 2 diabetes mellitus(T2DM)in the western community of Shenzhen.Methods A total of 200 patients with T2DM in the western community of Shenzhen from February 2020 to February 2021 were enrolled in the study.The current situation of mastering the foot care knowledge,and foot self-care behavior as well as influencing factors were analyzed.Moreover the patients were randomly divided into control group and research group.The patients in control group received routine nursing intervention,while those in research group received“knowledge,attitude and practice”(KAP)mode nursing,then the nursing results were observed and compared between the two groups.Results In the evaluation of the 200 patients in the community,the scores of mastering the foot nursing knowledge were(20.35±2.11)points,with medium level,and the standard scores were 71.20 points.And the scores of foot self-nursing behavior were(18.92±1.57)points,with poor results,and the standard scores were 58.21 points.The univariate analysis showed that the factors affecting the knowledge of foot care and self-care behavior included education level,duration of disease,health guidance and regular rechecking(P<0.05).Multiple stepwise linear regression analysis showed that the independent variables(influencing factors)were education level,duration of disease,health guidance and regular rechecking(P<0.05).Pearson correlation analysis showed that the scoring results of foot care knowledge were positively correlated with foot self-care behavior(P<0.01).Moreover the scores of mastering foot nursing knowledge and foot self-care behavior scores in research group were superior to those in control group(P<0.05).Conclusion At present,the current situation of mastering foot nursing knowledge mastery and foot self-care behavior in patients with T2DM in the western community of Shenzhen is poor,which is mainly affected by cultural
