脂类水平与成人细菌性社区获得性肺炎的关系

摘　　要：目的探讨成人细菌性社区获得性肺炎(CAP)与脂类代谢异常的关系。方法回顾性分析183例细菌性CAP患者的降钙素原(PCT)、红细胞沉降率(ESR)、C反应蛋白(CRP)、外周血白细胞(WBC)与血清脂类水平的相关性。结果PCT与血清胆固醇(CHOL)和低密度脂蛋白(LDL)呈负相关(r=-0.166,-0.223,P<0.05);ESR与血清高密度脂蛋白(HDL)和载脂蛋白A1(ApoA1)呈负相关(r=-0.437,-0.499,P<0.05);CRP与血清CHOL、HDL、LDL、ApoA1和ApoB呈负相关(r=-0.316,-0.273,-0.327,-0.433,-0.302,P<0.05);外周血WBC与血清ApoA1呈负相关(r=-0.181,P<0.05)。结论细菌性CAP患者炎症水平与血清脂类代谢密切相关,随着炎症水平的加重,血清CHOL、LDL、HDL、ApoA1和ApoB的水平有所下降。Objective To observe the correlation between inflammatory markers and lipid profiles in adult bacterial community-acquired pneumonia(CAP).Methods Data from 183 adult patients with bacterial CAP receiving medical treatment were analyzed retrospectively to learn the primary correlation between inflammatory markers and lipid profiles.Results The investigation revealed that PCT negatively correlated with serum total CHOL and LDL-cholesterol(r=-0.166,-0.223,P<0.05).Also,a negative correlation was discovered between ESR and serum HDL-cholesterol and ApoA1(r=-0.437,-0.499,P<0.05).CRP negatively correlated with serum CHOL,HDL-cholesterol,LDL cholesterol,ApoA1 and ApoB(r=-0.316,-0.273,-0.327,-0.433 and-0.302,P<0.05).Peripheral blood WBC negatively correlated with serum ApoA1(r=-0.181,P<0.05).Conclusions Lipid profiles correlates to the severity of illness in bacterial infection,and the level of serum CHOL,LDL-cholesterol,HDL-cholesterol,ApoA1 and ApoB decrease with aggravation of inflammation in bacterial CAP.
