摘 要:目的:研究我院住院患者中皮肤软组织感染者(SSTIs)金黄色葡萄球(SA)感染情况及MRSA与MSSA耐药特点。方法:选取我院2018年1月1日至2020年12月31日入院时存在SSTIs且进行细菌培养及药敏试验的患者,分析SA及MRSA检出率、病种分布,以及MRSA与MSSA耐药性的差异。结果:共分析1455例患者,其中检出SA 323例,占总数比例为22.2%;MRSA阳性者72例,占SA总数的22.3%。2018-2020年MRSA检出率呈逐年升高的趋势。MRSA阳性者主要病种来源为天疱疮、大疱性类天疱疮、湿疹、银屑病、红皮病等。MRSA对阿奇霉素、青霉素、红霉素、克拉霉素、四环素等11种抗菌药物的耐药率均显著高于MSSA(均P<0.05),而对复方新诺明、克林霉素、庆大霉素的耐药率与MSSA无明显差异(均P>0.05)。结论:我院SSTIs致病菌SA检出率高,MRSA在SA中占比高,且对大多数抗菌药物的耐药率显著高于MSSA。Objective: To study the infection situation of Staphylococcus aureus(SA) and drug resistance characteristics of MRSA and MSSA in inpatients that infected with skin and soft tissue infections(SSTIs) in our hospital. Methods: The data of SSTIs inpatients who underwent bacterial culture and drug sensitivity test in our hospital from January 1, 2018 to December 31, 2020 were collected and the recall factor and disease distribution of SA, MRSA and differences in drug resistance between MRSA and MSSA were analyzed. Results: A total of 1455 SSTIs inpatients were collected, including 323 SA infection(22.2%). Of 323 SA cases, 72(22.3%) were positive for MRSA. The recall factor of MRSA showed an increasing trend from 2018 to 2020. The patients positive for MRSA were most pemphigus, bullous pemphigoid, eczema, psoriasis and erythroderma. The drug resistance rates of MRSA to 11 antibacterial agents such as azithromycin, penicillin, erythromycin, clarithromycin and tetracycline were significantly higher than those of MSSA, with significant differences(P;<0.05), and there was no significant difference between MRSA and MSSA on drug resistance rates of cotrimoxazole, clindamycin and gentamicin(P;>0.05). Conclusion: The recall factor of SA in SSTIs is high in our hospital, and the proportion of SA in MRSA is high, and the drug resistance rate of MRSA to most antibacterial drugs is significantly higher than MSSA.
