妊娠期使用抑酸药物与儿童哮喘相关性的Meta分析

摘　　要：目的:对孕期使用抗酸药物与儿童哮喘之间的关系进行系统回顾和Meta分析,为目前的研究提供进一步的依据。方法:全面检索PubMed,EMBASE,Cochrane,Ovid以及中国知网、万方等数据库中关于妊娠期使用抑酸药物与儿童哮喘相关性的研究,检索时限截止2019年10月,采用Stata12.0软件,通过Meta分析方法探讨二者之间的关系。结果:在240篇筛选出的文章中,有8项研究符合纳入标准。当汇总纳入的研究时,妊娠期使用所有抑酸药物与儿童哮喘风险增加相关(OR=1.34,95%CI 1.17~1.54,I^(2)=81%,P<0.00001)。妊娠期使用质子泵抑制剂(PPI)会增加儿童患哮喘风险(OR=1.39,95%CI 1.21~1.59,I^(2)=44%,P<0.00001),妊娠期使用H_(2)受体拮抗剂(H_(2)RAs)者具有同样的结果(OR=1.51,95%CI 1.37~1.66,I^(2)=15%,P<0.00001)。结论:妊娠期使用抗酸药物会增加儿童患哮喘风险。这一结果可以提醒临床医生和父母谨慎使用抑酸药物。Objective:A systematic review and meta-analysis of the relationship between antacid use during pregnancy and childnood asthma provided further evidence for the current study.Methods:The database including PubMed,EMBASE,Cochrane,Ovid,CNKI and Wanfang were searched from inception to October in 2019.Among the data were studies of acid-suppressant use during pregnancy and asthma in children.Stata 12.0 software was used to explore the relationship between the two by meta analysis.Results:Of the 240 articles screened,eight of the studies met our inclusion criteria.When the included studies were pooled,the use of antacid medications during pregnancy was associated with an increased risk of childhood asthma(OR=1.34,95%CI 1.17~1.54,I^(2)=81%,P<0.00001).The overall risk of asthma in childhood increased among proton pump inhibitor(PPI)users(OR=1.39,95%CI 1.21~1.59,I^(2)=44%,P<0.00001)and histamine-2 receptor antagonist(H_(2)RAs)users(OR=1.51,95%CI 1.37~1.66,I^(2)=15%,P<0.00001).Conclusion:Taking acid-suppressing drugs during pregnancy can significantly increase a child's risk of developing asthma.This information could help clinicians and parents use acid-suppressing drugs with caution when deciding whether to take them during pregnancy.
