摘 要:目的分析不同病毒载量对乙型肝炎病毒(HBV)感染相关肝病患者肝功能、炎性因子和预后的影响。方法选择我院2019年8月至2021年4月收治的80例HBV感染相关肝病患者,以血清中HBV DNA拷贝数为依据,分为阴性组(0~10^(3) copies/mL,n=21)及低(10^(3)~10^(5) copies/mL,n=20)、中(10^(5)~10^(7) copies/mL,n=21)、高(>10^(7) copies/mL,n=18)拷贝组,对比四组患者的肝功能指标[丙氨酸氨基转移酶(ALT)、γ-谷氨酰转移酶(GGT)、天门冬氨酸氨基转移酶(AST)]、炎性因子指标[肿瘤坏死因子-γ(INF-γ)、白介素-21(IL-21)]水平及预后。结果随着病毒载量增加,ALT、GGT、AST、INF-γ、IL-21水平逐渐升高,患者预后良好率逐渐降低(P<0.05)。结论HBV感染相关肝病患者肝功能指标、炎性因子水平随着病毒载量增加而升高,进而对预后产生影响。Objective To analyze the influence of different viral loads on liver function,inflammatory factors and prognosis of patients with hepatitis B virus(HBV)infection-related liver diseases.Methods 80 patients with HBV infection-related liver diseases admitted to our hospital from August 2019 to April 2021 were selected and divided into negative group(0-10^(3) copies/mL,n=21),and low(10^(3)-10^(5) copies/mL,n=20),medium(10^(5)-10^(7) copies/mL,n=21)and high(>10^(7) copies/mL,n=18)copy groups according to the serum HBV DNA copies.The liver function indicators[alanine aminotransferase(ALT),γ-glutamyltransferase(GGT),aspartate aminotransferase(AST)],inflammatory factor indicators[tumor necrosis factor-γ(INF-γ),Interleukin-21(IL-21)]and prognosis in the four groups were compared.Results With the increase of viral loads,the levels of ALT,GGT,AST,INF-γand IL-21 gradually increased,and the good prognosis rate of patients gradually decreased(P<0.05).Conclusions With the increase of viral loads,the levels of liver function indicators and inflammatory factors in patients with HBV infection-related liver diseases increase,and have an impact on the prognosis.
