摘 要:目的探讨海昆肾喜胶囊联合氯沙坦钾片治疗慢性肾脏病患者的疗效及对肾功能和炎性因子的影响。方法选择绍兴市中心医院2018年1月至2019年12月诊治的慢性肾脏病患者100例,按随机数字表法分为观察组和对照组,每组50例。对照组在常规治疗的基础上口服氯沙坦钾片治疗,观察组在对照组治疗的基础上口服海昆肾喜胶囊,两组疗程均为12周。比较两组治疗后的疗效和治疗前后肾功能指标、炎性因子、24 h尿蛋白定量(24 h Upro)和肾小球滤过率(GFR)的变化。结果观察组治疗后总有效率高于对照组[90.0%(45/50)比72.0%(36/50)],差异有统计学意义(χ^(2) = 5.26,P<0.05)。两组治疗后肾功能指标血清肌酐(Scr)和尿素氮(BUN)水平低于治疗前,并且观察组治疗后Scr和BUN水平低于对照组[(63.27 ± 2.89) μmol/L比(67.89 ± 2.35) μmol/L、(5.23 ± 0.19) mmol/L比(5.56 ± 0.16) mmol/L],差异有统计学意义(P<0.05)。两组治疗后炎性因子C反应蛋白(CRP)、白细胞介素6(IL-6)和肿瘤坏死因子-α(TNF-α)水平低于治疗前,并且观察组治疗后CRP、IL-6和TNF-α水平低于对照组[(2.97 ± 0.34) mg/L比(3.58 ± 0.42) mg/L、(3.64 ± 0.68) ng/L比(4.97 ± 0.96) ng/L、(14.32 ± 2.17) ng/L比(17.86 ± 2.06) ng/L],差异有统计学意义(P<0.05)。两组治疗后24 h Upro低于治疗前,而GFR高于治疗前,并且观察组治疗后24 h Upro低于对照组[(0.87 ± 0.09)g比(1.15 ± 0.13)g],而GFR高于对照组[(101.73 ± 3.12)ml/(min·m^(2))比(96.75 ± 2.35)ml/(min·m^(2))],差异均有统计学意义(P<0.05)。结论海昆肾喜胶囊联合氯沙坦钾片治疗慢性肾脏病患者疗效良好,并且可改善患者肾功能和微炎性反应状态。Objective To investigate the curative effect of Haikunshenxi capsule combined with losartan potassium tablets on chronic kidney disease(CKD)and its effect on renal function and inflammatory factors.Methods One hundred patients with chronic kidney disease in Shaoxing Central Hospital from January 2018 to December 2019 were selected and randomly divided into observation group(50 cases)and control group(50 cases).The control group was treated with losartan potassium tablets based on conventional therapy,and the observation group was treated with Haikunshenxi capsulebase on control group.The treatment course of the two groups was 12 weeks.The curative effect,renal function,inflammatory factors,24h urinary protein(24 h Upro)and glomerular filtration rate(GFR)were compared between the two groups before and after treatment.Results The total effective rate in the observation group was higher than that in the control group:90.0%(45/50)vs.72.0%(36/50),χ^(2)=5.26,P<0.05.After treatment,the levels of serum creatinine(Scr)and blood urea nitrogen(BUN)in the two groups were decreased and the levels of Scr and BUN in the observation group were lower than those in the control group:(63.27±2.89)μmol/L vs.(67.89±2.35)μmol/L,(5.23±0.19)mmol/L vs.(5.56±0.16)mmol/L,the differences were statistically significant(P<0.05).After treatment,the levels of serum C-reactive protein(CRP),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in the two groups were decreased and the levels of CRP,IL-6 and TNF-αin the observation group were lower than those in the control group:(2.97±0.34)mg/L vs.(3.58±0.42)mg/L,(3.64±0.68)ng/L vs.(4.97±0.96)ng/L,(14.32±2.17)ng/L vs.(17.86±2.06)ng/L,the differences were statistically significant(P<0.05).After treatment,the level of 24 h Upro in two groups was decreased,while the level of GFR was increased,and the level of 24 h Upro in the observation group was lower than that in the control group:(0.87±0.09)g vs.(1.15±0.13)g,but the level of GFR in the observation group was higher than that in the
