血清SFRP 5、脂联素水平变化与妊娠期糖尿病患者HOMA-IR的关联性

摘　　要：目的探究血清分泌型卷曲相关蛋白5(SFRP 5)、脂联素水平变化与妊娠期糖尿病(GDM)患者稳态模型评估-胰岛素抵抗指数(HOMA-IR)的关联性。方法选择2018年1月-2019年10月本院收治的GDM患者89例作为研究组,选同期健康体检妊娠者85名作为对照组,检测两组孕妇血清SFRP 5、脂联素水平,并计算HOMA-IR,分析血清SFRP 5、脂联素水平变化与HOMA-IR的关联性。结果研究组孕早期、孕中期血清SFRP 5水平、血清脂联素水平低于对照组,HOMA-IR大于对照组(P<0.05);经Pearson分析,孕早期、孕中期血清SFRP 5水平(r_(早)=-0.524;r_(中)=-0.549)、血清脂联素水平(r_(早)=-0.626;r_(中)=-0.498)与HOMA-IR呈负相关(P<0.05);经Logistic分析,孕早期血清SFRP 5水平<10.5 ng ml^(-1)、孕中期血清SFRP 5水平<6.8 ng ml^(-1)、孕早期血清脂联素水平<6.1 mg L^(-1)、孕中期血清脂联素水平<5.2 mg L^(-1)均为发生GDM独立危险因素(P<0.05)。结论GDM患者体内的血清SFRP 5、脂联素水平降低,并与HOMA-IR呈负相关,此外,实时监测血清SFRP 5、脂联素水平,利于早期发现胰岛素抵抗。Objective To explore the relationship between changes in serum secreted frizzled-related protein 5(SFRP 5),adiponectin levels and homeostasis model assessment-insulin resistance index(HOMA-IR)in patients with gestational diabetes mellitus(GDM).Methods A total of 89 GDM patients those were admitted to our hospital from January 2018 to October 2019 were selected as the research group,and 85 pregnant women receiving healthy physical examination during the same period were selected as the control group.The serum SFRP 5 and adiponectin levels of the two groups were detected and HOMA-IR was calculated,the correlation between changes in serum SFRP 5,adiponectin levels and HOMA-IR was analyzed.Results The serum SFRP 5 levels and serum adiponectin levels of the study group were lower than those of the control group in the first and second trimesters,and HOMA-IR was higher than that of the control group(P<0.05).According to Pearson analysis,in the early and metaphase pregnancy,the level of serum SFRP 5(rearly=-0.524;rmid-term=-0.549)and serum adiponectin level(rearly=-0.626;rmid-term=-0.498)were negatively correlated with HOMA-IR(P<0.05).Logistic analysis showed that the serum SFRP 5 level lower than10.5ng.ml^(-1)in the first trimester,serum SFRP 5 level lower than 6.8ng.ml^(-1) in the second trimester,serum adiponectin level lower than 6.1 mg.L^(-1)in the early trimester,serum Adiponectin levels lower than 5.2 mg.L^(-1)in the second trimester were all independent risk factors for GDM(P<0.05).Conclusions The levels of serum SFRP 5 and adiponectin in patients with GDM are reduced,and are negatively correlated with HOMA-IR.In addition,real-time monitoring of serum SFRP 5 and adiponectin levels is helpful for early detection of insulin resistance.
