空心螺钉联合内侧buttress钢板治疗PauwelsⅢ型青少年股骨颈骨折疗效分析

摘　　要：目的:观察空心螺钉联合内侧buttress钢板治疗PauwelsⅢ型青少年股骨颈骨折的临床疗效。方法:回顾性分析2019年1月至2020年1月本院收治的48例PauwelsⅢ型青少年股骨颈骨折患者,根据内固定方式分为两组,A组(n=25)采用3枚空心加压钉内固定;B组(n=23)采用3枚空心加压钉联合内侧buttress钢板内固定,术后定期复查、随访。比较两组患者手术切口长度、术中出血量、手术时间、术后髋关节harris评分、VAS疼痛评分、garden指数及术后并发症。结果:所有患者均获得随访,随访时间12〜18个月。①两组手术切口长度比较无明显差异(P>0.05);A组术中出血量、手术时间均小于B组(P<0.05);②B组术后各时点髋关节hands评分均高于A组(P<0.05);③两组术前VAS评分比较无明显差异(P>0.05),B组术后各时点VAS评分均低于A组(P<0.05);④两组术后初次正侧位片garden指数比较无明显差异(R>0.05),B组术后6个月正侧位片garden指数明显高于A组(P<0.05);⑤B组术后并发症发生率低于A组(P<0.05)。结论:采用空心螺钉联合内侧buttress钢板治疗PauwelsⅢ型青少年股骨颈骨折能获得并维持满意复位,内固定强度高,术后功能恢复好,利于患者预后康复。Objective:To observe the clinical ffect of cannulated screw combined with medial buttress plate in the treatment of Pauwels Ⅲ adolescent femoral neck fracture.Methods:From January 2019 to January 2020,48 cases of Pauwels Ⅲ adolescent femoral neck fracture were divided into group A and group B according t0 the internal fixation method.25 patients in group A were treated with 3 hollow compression screws for intermal fixation,and 23 patients in group B were treated with 3 hollow compression screws combined with medial buttress plate for intermal fixation.Regular review and fllow-up after surgery were carried out in all patients.meanwhile,the length of incision,intraoperative blood loss,operation time,postoperative Harris score of hip joint,VAS pain score,garden index and postoperative complications were compared between the two groups.Results:All patients were ollowed up for 12-18 months.①There was no significant diference in the length of incision between two groups(P>0.05).Intraoperative blood loss and operation time in group A were less than those in group E(P<0.05).②Hip Harris scores of group B were higher than those of group A at each time point after operation(P<0.05).③There was no significant diference in the preoperative VAS score between the two groups(P>0.05),and the VAS score in group B was lower than that in group A at each time point after operation(P<0.05).④There was no significant difference in the garden index of the first anterior and lateral view between the two groups after surgery(P>0.05).The garden index of frontal and lateral radiographs in group B wap significantly higher than that in group A at 6 months after operation(P<0.05).⑤The incidence of postoperative complications in group B was lower than that in group A(P<0.05).Conclusion:Cannulated screw combined with medial buttress plate in the treatment of Pauwels Ⅲ adolescent femoral neck fracture can obtain and maintain a satisfactory reduction,with high intermal fixation strength and good postoperative functional recovery,whic
