摘 要:目的探讨T_(1)FLAIR增强序列在脑膜结核MRI扫描中的参数优化和应用价值,提高病变检出率。方法应用GE Signa HD XT_(1).5T MR扫描仪,将90例结核性脑膜炎患者随机分成三组,在进行常规扫描后注射对比剂,分别应用fs SE T_(1)WI序列及3组不同参数组合的fs T_(1)FIAIR序列中的一种进行横断面增强扫描,对比3组图像信噪比(SNR)、分辨率、脑白质T_(1)对比度,选取最佳T_(1)FLAIR扫描参数组合;将最佳T_(1)FLAIR扫描参数组合图像与常规增强fs SE T_(1)WI序列图像进行比较,评估两者的对比噪声比(CNR),评价其在显示脑膜结核病变及病变与脑膜解剖结构关系方面的优势。结果在1.5T场强下,应用3种不同参数组合的fs T_(1)FLAIR序列增强扫描,使用单因素方差分析(ANOVA分析),组间两两比较提示2组SNR与1组SNR、2组SNR与3组SNR之间均有明显统计学差异(P<0.01)。经比对,第二组参数组合TR 2050 ms;TE 16 ms;TI 800 ms;NEX 4次;矩阵320×192;应用ASSET及fs技术的情况下,可以达到提高图像T_(1)对比度及SNR的效果。使用配对t检验比较fs SE T_(1)WI序列和fs T_(1)FLAIR序列的CNR,提示两组CNR差异有统计学意义(P<0.01),表明该序列在脑膜结核病变影像显示方面与fs SE T_(1)WI序列比较,在对比噪声比方面更具优势。结论在脑膜结核病变的扫描中,fs T_(1)FLAIR增强序列的作用十分重要,通过参数优化,可以提高图像信噪比;优化参数的fs T_(1)FLAIR增强序列可以取得较常规扫描的fs SE T_(1)WI序列更满意的图像效果,在脑膜结核病变显示以及病变与脑灰白质、脑沟回的解剖结构关系显示方面具有优势。Objective To explore the parameter optimization and its application value of T_(1)FLAIR enhanced sequence of MRI in meningeal tuberculosis,so as to improve the detection rate of lesions.Methods Using GE signa HD XT_(1).5T MR scanner,90 patients with tuberculous meningitis were randomly divided into three groups.After routine scanning,contrast medium was injected,and FS se T_(1)WI sequence and one of the three groups of FS T_(1)fiair sequences with different parameter combinations were used for cross-sectional enhancement scanning.The SNR,resolution and T_(1)contrast of cerebral white matter of the above three groups were compared and evaluated.We selected and applied the best combination of T_(1)FLAIR scanning parameters to improve the detection rate of lesions,and then evaluated the CNR compared with the enhanced fs SE T_(1)WI sequence image,indicating its advantages in highlighting the relationship between the lesions of meningeal tuberculosis and meningeal anatomy.Results Under the field strength of 1.5 T,three groups of FS T_(1)FLAIR sequences with different parameter combinations were used.Using ANOVA analysis,pairwise comparison between two groups showed that there were significant statistical differences between group 2 of SNR and group 1 of SNR,group 2 of SNR and group 3 of SNR(all P values were 0.000).Via comparative analysis,satisfactory imaging results can be obtained with the second group of parameter combinations(TR 2050 ms;TE 16 ms;TI 800 ms;NET 4 times;matrix 320×192)under the application of ASSET and fs technology,which showed its advantages in the display of meningeal tuberculosis lesions,the distinction between lesions and gray matter,and the display of the anatomic structure of the cerebral sulcus.Paired t test was used to compare CNR of FS se T_(1)WI and FS T_(1)FLAIR sequences,indicating that there was significant difference in CNR between the two groups(P=0.000).Besides,compared with fs SE T_(1)WI,the sequence had more advantages in the imaging display of meningeal tuberculosis,which can in
