摘  要:目的探讨三维多普勒能量血管成像(3D-CPA)及其定量参数在青春期多囊卵巢综合征(PCOS)患者中的应用价值,旨在为青春期PCOS的诊断提供技术参考。方法选取2018年7月至2020年3月在本院就诊的79例疑似青春期PCOS患者为观察对象,所有患者均接受常规二维超声、性激素检查;参照相关标准,结合临床检查结果将确诊为PCOS的患者纳入PCOS组,其他患者纳为非PCOS组,全部患者均接受3D-CPA检查;比较两组血管指数(VI)、血流指数(FI)、血管化血流指数(VFI),并分析VI、FI、VFI用于青春期PCOS的诊断价值。结果79例疑似青春期PCOS患者参照相关标准评估结果显示,34例确诊为青春期PCOS,占43.04%;PCOS患者3D-CPA成像特点:可见长棒状或树枝状血流信号,数量丰富,且多在卵巢髓质位置,血流起源于卵巢门;PCOS组VI、FI、VFI均高于非PCOS组,差异有统计学意义(P<0.05);将青春期PCOS确诊情况作为状态变量,将VI、FI、VFI作为检验变量,绘制ROC曲线,结果显示,VI、FI、VFI诊断PCOS的AUC分别为0.878、0.854、0.842,AUC均>0.80,均有一定诊断价值,当VI、FI、VFI cut-off值分别取4.805、24.330、2.035时,可获得最佳诊断价值。结论3D-CPA技术对青春期PCOS有一定诊断价值,其相应定量参数可预测青春期PCOS发生风险,未来可考虑用3D-CPA技术辅助评估青春期PCOS发生风险,可能对疾病的早期干预诊断及治疗指导有积极意义。Objective To investigate the value of three-dimensional Doppler power angiography(3D-CPA)and its quantitative parameters in adolescent patients with polycystic ovary syndrome(PCOS),aiming to provide technical reference for the diagnosis of adolescent PCOS in the future.Methods A total of 79 patients with suspected adolescent PCOS in our hospital from July 2018 to March 2020 were selected as the research objects.All patients received examination of conventional two-dimensional ultrasound and sex hormone.According to the relevant standards and clinical examination results,the patients with PCOS were included in the PCOS group,and other patients were included in the non PCOS group.All patients underwent 3D-CPA examination.The vascular index(VI),blood flow index(FI)and vascularized blood flow index(VFI)were compared between the two groups.The diagnostic value of VI,FI and VFI in adolescent PCOS was analyzed.Results Among 79 patients with suspected adolescent PCOS,34 cases were diagnosed as adolescent PCOS,accounting for 43.04%;As for 3D-CPA imaging features of PCOS patients,long rod or tree like blood flow signals could be seen,which were abundant,and for most of them were located in the ovarian medulla,and the blood flow originated from the ovarian hilum,the levels of VI,FI and VFI in PCOS group were higher than those in non PCOS group(P<0.05);when the diagnosis of adolescence PCOS was taken as the state variable,VI,FI and VFI were taken as the test variables,and the ROC curve was drawn,the AUC of FI and VFI in the diagnosis of PCOS were 0.878,0.854 and 0.842,respectively,all AUC were>0.80,when the cut-off values of VI,FI and VFI were 4.805,24.330 and 2.035 respectively,the best diagnostic value could be obtained.Conclusion 3D-CPA has certain diagnostic value for adolescent PCOS,and its quantitative parameters can predict the risk of adolescent PCOS,3D-CPA technology can be considered to assess the risk of adolescence PCOS in the future,which may have positive significance for the guidance of early intervention diag
