摘 要:目的研究眼眶炎性假瘤的常规超声和超声造影的表现特点。方法回顾性病例研究。分析2012年2月至2015年8月我院诊治的40例(40只眼)眼眶炎性假瘤患者的常规超声和超声造影检查资料,应用SonoLiver软件分析超声造影资料,以炎性假瘤和正常眼眶组织进行对照分析获得动态血管模式(DVP)曲线、时间-强度(TI)曲线及相关定量数据。结果炎性假瘤常规超声表现为内部呈混合回声或低回声,其内可探及较丰富或少许彩色血流信号;超声造影DVP曲线多呈正向波形;TI曲线多呈快进慢出模式,即造影剂填充早于正常眼眶组织,内呈低-中等增强,造影剂消退晚于正常眼眶组织。定量数据:峰值强度(PI)、平均渡越时间(mTT)炎性假瘤组高于对照组,上升时间(RT)、达峰时间(TTP)炎性假瘤组低于对照组,差异具有统计学意义(P<0.05)。结论眼眶炎性假瘤的常规超声和超声造影表现具有特异性,有助于眼眶炎性假瘤的诊断和鉴别诊断。Objective To investigate the features of orbital inflammatory pseudotumor using conventional ultrasound and contrast-enhanced ultrasound.Methods In this retrospective cases series,40 eyes of the 40 patients who were diagnosed with orbital inflammatory pseudotumor and underwent conventional ultrasound and contrast-enhanced ultrasound examination in the Fourth People’s Hospital of Shenyang were collected from February 2012 to August 2015.The sonoliver software was used to analyze contrast-enhanced ultrasound data.The dynamic vascular pattern(DVP)curve,time-intensity(TI)curve and related quantitative data were obtained by comparing inflammatory pseudotumor with normal orbital tissue.Results The conventional ultrasound manifestations of inflammatory pseudotumor were mixed echo or hypoecho,and abundant or a few color blood flow signals could be detected.The DVP curve of contrast-enhanced ultrasound(CEUS)was mostly positive waveform.The TI curve mostly showed a fast in and slow out pattern,that is,the filling of the contrast agent was earlier than that of normal orbital tissue,and the internal enhancement was low to medium.The contrast agent subsided later than that of normal orbital tissue.Quantitative data:maximum intensity(PI)and mean transit time(MTT)in the inflammatory pseudotumor group were higher than that in the control group,while rising time(RT)and time to peak(TTP)in the inflammatory pseudotumor group were lower than that in the control group,and the difference were statistically significant(P<0.05).Conclusions Conventional ultrasound and contrast-enhanced ultrasound features of orbital inflammatory pseudotumor are specific,which are valuable tools for the diagnosis and differential diagnosis of orbital inflammatory pseudotumor.
