摘 要:目的探讨miR-106b-5p和miR-202-3p在弱精子症和非梗阻性无精子症(NOA)患者精浆中的表达情况及其临床价值。方法选取2018年12月至2020年10月于唐山市妇幼保健院就诊的男性不育症患者和精液参数正常的志愿者为研究对象,根据精液检查参数和临床资料不同分为3组:精子浓度>15×10^(6)/ml且精子前向运动(PR)<32%为弱精子症组(n=106),NOA患者为NOA组(n=92),同期精液检查参数正常的男性为对照组(n=119)。提取各组研究对象的精浆miRNA,采用实时定量PCR(qRT-PCR)法分析各组对象精浆miR-106b-5p和miR-202-3p的表达情况;采用受试者工作特征曲线(ROC)评价精浆miR-106b-5p和miR-202-3p对NOA的诊断价值。结果qRT-PCR检测结果显示,NOA组miR-106b-5p和miR-202-3p的表达水平显著低于对照组及弱精子症组(P<0.05),而弱精子症组中两者的表达水平与对照组比较无显著性差异(P>0.05)。ROC分析结果显示,miR-106b-5p诊断NOA的ROC曲线下面积(AUC)为0.750[95%CI(0.665,0.835),P<0.0001],灵敏度和特异性分别为72.1%和61.2%;miR-202-3p诊断NOA的AUC为0.873[95%CI(0.810,0.936),P<0.0001],灵敏度和特异性分别为88.6%和82.9%。结论NOA患者精浆中miR-106b-5p和miR-202-3p的表达显著降低,二者可能成为NOA临床诊断的新标志物。Objective:To explore the expression of miR-106 b-5 p and miR-202-3 p in the seminal plasma of asthenospermia and non-obstructive azoospermia(NOA) patients and its clinical significance.Methods:Male infertile patients and volunteers with normal semen parameters were included in Maternity and Child Health Care Hospital of Tangshan from December 2018 to October 2020. The patients were divided into three group according to semen examination parameters and clinical data: the patients with sperm concentration >15×10^(6)/ml and sperm forward movement(PR)<32% were the asthenospermia group(n=106),NOA group(n=92),and men with normal semen in the same period were the control group(n=119). The seminal plasma miRNA was extracted, and real-time quantitative PCR(qRT-PCR) was used to analyze the expression of miR-106 b-5 p and miR-202-3 p. The receiver operating characteristic curve(ROC) was drawn to evaluate the diagnostic value of miR-106 b-5 p and miR-202-3 p for NOA.Results:The qRT-PCR test results showed that the expression levels of miR-106 b-5 p and miR-202-3 p in the NOA group were significantly lower than those in the control group and asthenospermia group(P<0.05). The expression levels of miR-106 b-5 p and miR-202-3 p in the asthenospermia group were not significantly different with the control group(P>0.05).ROC curve analysis showed that the area under curve(AUC) of miR-106 b-5 p was 0.750[95%CI(0.665,0.835),P<0.000 1],and the sensitivity and specificity were 72.1% and 61.2% respectively. The AUC of miR-202-3 p was 0.873[95%CI(0.810,0.936),P<0.000 1],and the sensitivity and specificity were 88.6% and 82.9% respectively.Conclusions:The expression of miR-106 b-5 p and miR-202-3 p in seminal plasma of patients with NOA decreases significantly. The expression of miR-106 b-5 p and miR-202-3 p in seminal plasma may be a new marker for clinical diagnosis of NOA.
