摘 要:目的 制备一种可直接用于起搏器依赖患者的高频电刀系统并验证应用于临床的可行性及安全性。方法 高频电刀主机采用上海沪通GD350-B系统,心电R波控制器采用深圳朗锐慧康心电采集模块,控制模块采用意法半导体(ST)公司的32位单片机STM32F103C8T6,与心电采集模块通过串口进行通信,心电数据经过处理后,由R波同步触发高频电刀输出。测定高频电刀启动信号及输出与R波的延时。验证研究入选更换起搏器的患者10例,其中房室传导阻滞6例,病窦综合征4例,起搏器程控仪接体表心电图,测定电刀输出与起搏器心室起搏(VP)脉冲及心室感知(VS)的延时间期,分析触发噪音反转(NR)的成功率。结果 高频电刀启动信号及输出与R波的延时分别为5.6 ms及33.6 ms。电刀输出与起搏器VP脉冲及VS的延时分别为(128.70±20.22) ms及(113.60±22.67) ms。触发NR成功率为100%。结论 R波触发型高频电刀系统可触发起搏器NR,安全地直接用于起搏器依赖的患者。Objective A high frequency electrotome system which can be directly used in pacemaker dependent patients is prepared and its clinical feasibility and safety are verified. Methods The high frequency electrotometer host adopted Shanghai Hutong GD350-B system, the electrocardiogram(ECG) R wave trigger used Shenzhen Loongray Huikang ECG acquisition module, and the control module used ST M32 F103 C8 T6, a 32-bit single-chip microcomputer of ST Microelectronics, to communicate with the ECG acquisition module through serial port. After the ECG data was processed, the high frequency electrotometer was triggered by R wave synchronicity. The start signal and delay between output of high frequency electrotome and R wave were measured. Validation study included 10 patients with pacemaker replacement, including 6 patients with atrioventricular block, 4 patients with sick sinus syndrome, pacemaker programmable controller connected to body surface electrocardiogram, measured the delay period between electrotome output and pacemaker ventricular pacing(VP) pulse and ventricular sensing(VS), and analyzed the success rate of triggering noise reversion(NR). Results The start signal and output delay of the high frequency electrotome and the R wave were 5.6 ms and 33.6 ms respectively. The delay from electrotome output to pacemaker VP pulse and VS were(128.70±20.22) ms and(113.60±22.67)ms respectively, and NR was successfully performed 100%. Conclusion R-wave triggered high-frequency electrotome system can trigger pacemaker NR, which can be directly and safely used in pacemaker dependent patients.
