益气通脉方联合芒硝外敷治疗膝关节置换术后下肢肿胀疗效观察

摘　　要：目的观察益气通脉方联合芒硝外敷对膝关节置换(TKA)术后下肢肿胀疗效。方法将我院80例TKA术后下肢肿胀患者随机分为观察组(在常规功能锻炼基础上联合益气通脉方、芒硝外敷治疗)与对照组(常规功能锻炼)各40例。观察两组患者临床疗效,治疗前后下肢肿胀中医证候积分,患肢髌上、下10cm周径及视觉模拟评分(VAS)、膝关节屈曲度。结果治疗后,观察组总有效率显著高于对照组(P<0.05);两组皮肤瘀斑、皮肤发红、肿胀、局部红疹、局部发热积分均下降,观察组皮肤瘀斑、皮肤发红、肿胀、局部红疹、局部发热积分均低于对照组(P<0.05);两组肢髌上、下10cm周径及VAS评分均下降,膝关节屈曲度增高,观察组肢髌上、下10cm周径及VAS均低于对照组,膝关节屈曲度高于对照组(P<0.05)。结论益气通脉方联合芒硝外敷能显著改善TKA术后下肢肿胀患者的中医证候,缓解下肢肿胀,降低疼痛,增加膝关节屈曲度,疗效满意。Objective To observe the effect of Yiqi Tongmai Recipe combined with external application of mirabilite salt on lower limb swelling after knee arthroplasty(TKA).Methods Eighty patients with lower extremity swelling after TKA were selected and randomly divided into observation group(on the basis of routine functional exercise combined with Yiqi Tongmai Recipe and mirabilite external application treatment)and control group(routine functional exercise)each 40 cases.The clinical efficacy,the scores of TCM syndromes of lower extremity swelling before and after treatment,the 10cm circumference of the upper and lower patella of the affected limb,the Visual Analogue Scale(VAS)score,and the degree of knee flexion of the two groups were observed.Results After treatment,the total effective rate of observation group was significantly higher than that of control group(P<O.O5);the scores of skin ecchymosis,skin redness,swelling,local rash and local fever decreased in both groups;the scores of skin ecchymosis,skin redness,swelling,local rash and local fever in observation group were lower than those in control group(P<0.05);the upper and lower 10cm circumference and VAS score of both groups decreased,and the knee flexion degree increased;the upper and lower 10cm circumference and VAS score of observation group were lower than those of control group,and the knee flexion degree was higher than that of control group(P<0.05).Conclusion Yiqi Tongmai Recipe combined with external application of mirabilite can significantly improve the TCM syndromes of patients with lower extremity swelling after TKA,relieve lower extremity swelling,reduce pain,and increase knee flexion.The effect is satisfactory.
