活血祛痰方联合二甲双胍对高血压肥胖患者中医症候及血压变异性的影响

摘　　要：目的观察活血祛痰方联合二甲双胍对高血压肥胖患者中医症候及血压变异性的影响。方法将我院收治的160例高血压肥胖患者随机分为观察组与对照组各80例。观察组口服活血祛痰方+二甲双胍治疗,对照组口服二甲双胍,比较两组临床疗效,治疗前后中医症候总积分、血压变异性、体质量指数、空腹血糖、血脂水平。结果治疗后,观察组总有效率显著高于对照组(P<0.05);两组中医症候总积分下降,且观察组中医症候总积分低于对照组(P<0.05);两组24h、日间、夜间收缩压变异性,24h、日间舒张压变异性均下降,且观察组24h、日间、夜间收缩压变异性,24h、日间舒张压变异性均低于对照组(P<0.05);两组夜间舒张压变异性比较无明显差异(P>0.05);两组体质量指数(BMI)、空腹血糖(FBG)、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇LDL-C)均下降,高密度脂蛋白胆固醇(HDL-C)水平增高,且观察组BMI、FBG、TC、TG、LDL-C水平均低于对照组,HDL-C水平高于对照组(P<0.05)。结论活血祛痰方联合二甲双胍治疗高血压肥胖患者疗效显著,能改善临床症状,降低血压变异性,调节血脂水平。Objective To observe the effect of Huoxue Qutan Decoction combined with metformin on TCM symptoms and blood pressure variability in patients with hypertension and obesity.Methods 160 patients with hypertension and obesity were randomly divided into observation group and control group with 80 cases each.Observation group was treated with Huoxue Qutan Decoction combined with metformin,and control group was treated with metformin.The clinical effects of the two groups were compared,including TCM syndrome total score,blood pressure variability,body fat index,fasting blood glucose,and blood lipid levels before and after treatment.Results After treatment,the total effective rate of observation group was significantly higher than that of control group(P<0.05);the total score of TCM syndromes in both groups decreased,and was lower in observation group than control group(P<0.05);night systolic blood pressure variability,24h,day diastolic blood pressure variability decreased,and were lower in observation group than control group(P<0.05);there was no significant difference in the variability of nocturnal diastolic blood pressure between the two groups(P>0.05);body mass index(BMI),fasting blood glucose(FBG),total cholesterol(TC),triglycerides(TG),low-density lipoprotein cholesterol(LDL-C)decreased,while highdensity lipoprotein cholesterol(HDL-C)increased in both groups,and the BMI,FBG,TC,TG,LDL-C of observation group were lower than those of control group,and HDL-C was higher than that of control group(P<0.05).Conclusion Huoxue Qutan Decoction combined with metformin has a significant effect in treatment of patients with hypertension and obesity,which can improve clinical symptoms,reduce blood pressure variability,and regulate blood lipid levels.
