神经性皮炎的针灸诊疗特点研究

摘　　要：目的祖国传统医学认为神经性皮炎隶属于“顽癣”、“摄领疮”范畴。常被称作慢性单纯性苔藓。多表现为局限性病变,颈部、腕部、骶尾部、踝部为其好发部位,亦可见于眼睑、腰背部、四肢、外阴等部位。目前临床治疗本病多选择针灸疗法,安全性高、疗效好、无明显不良反应,在临床上得到广泛的应用。因此,探讨神经性皮炎的针灸诊疗特点及相关规律,有着重要的临床意义。方法分别检索中国知网(1959—2021年)、中文万方数据库(1983—2021年)、PUBMED(1974—2021年)中有关针灸治疗神经性皮炎的报道,根据文献检索结果,分析针灸辨证施治特点,治疗过程中的选穴依据及干预措施,同时总结针灸频率、总疗程、疗效(近期及远期)、随访治疗效果及安全性评价。结果对纳入的文献进行统计分析,神经性皮炎采用针灸治疗时以辨病为主,与辨证配穴相结合,取穴以皮损局部皮肤、曲池、三阴交、风池、足三里、血海等穴位为主;用梅花针在皮损部位局部叩刺,直至局部皮肤充血或少量出血,同时辅以灸法最常见;治疗总疗程为4~8周,频率大多采用隔日1次。结论神经性皮炎采用针灸治疗的近期疗效较好,部分病例远期疗效尚可,且安全性高,症状可明显缓解。Objective Neurodermatitis is also termed as chronic lichen simplex.It pertains to the category of psoriasis,ingestion sore and persistent tinea in TCM.On the whole,it appears in both sides of neck,elbow fossa,popliteal fossa,sacrococcygeal,wrist and ankle,as well as in waist,back,eyelid,limbs and vulva.The existing experience of acupuncture and moxibustion for treating neurodermatitis is increasingly abundant,and the treatment exhibits advantages of high curative effects,safety and non-toxic side effects.Such a treatment is critical to examine the characteristics and regularity of acupuncture and moxibustion for treating neurodermatitis.Methods Three databases,i.e.,CNKI(1959 to 2021),Wanfang Data(1983 to 2021)and PUBMED(1974 to 2021),were searched for articles on acupuncture for treating neurodermatitis,to analyze and summarize the common acupuncture intervention measures,the syndrome differentiation characteristics,the acupoint selection,the acupuncture frequency,the treatment course,the efficacy,the follow-up and the safety.Results In 18 articles,the diagnosis dependent on symptomacts as the main process of neurodermatitis treated by acupuncture,associated with TCM experiences to select acupoints among skin lesions,Xuehai,Quchi,Zusanli,Sanyinjiao,Fengchi,etc.Comprehensive treatment has been the most frequent method in acupuncture and moxibustion for treating neurodermatitis,especially the plum-blossom needle and moxibustion(employing the plum-blossom needle to tap the skin lesions till the blood is oozed out,and then applying it with moxibustion).The frequency of acupuncture is generally once every other day,with the total course of treatment about 4~8 weeks.Conclusion Acupuncture exerts a significant short-term effect on neurodermatitis,and it can alleviate the clinical symptoms.Moreover,acupuncture may exert a long-term effect to improve patients’health as safety methods.
