摘  要:目的探讨急性心肌梗死合并高血压患者早期脉压水平与心功能指标的相关性。方法收集衢州市人民医院心血管内科急性心肌梗死合并高血压患者124例,根据脉压水平将患者分为低脉压组(68例)和高脉压组(56例),左室射血分数(LVEF)<50%纳入低LVEF水平组,比较组间参数差异并行相关性分析及回归分析。结果低脉压组与高脉压组相比,BNP水平更高,LVEF水平更低,无心力衰竭征象(Killip分级I级)者更少,差异均有统计学意义(P<0.05);脉压与LVEF水平间呈正相关(r=0.242,P=0.007),脉压与BNP水平间呈负相关(r=-0.192,P=0.033);脉压降低及BNP水平升高是LVEF降低的独立危险因素(P<0.05)。结论早期脉压水平降低可预示BNP水平升高,对预测AMI合并高血压患者继发左心室功能障碍和心力衰竭具有重要的临床价值。Objective To investigate the correlation between early pulse pressure and cardiac function indexes in patients with acute myocardial infarction(AMI)and hypertension.Methods One hundred and twenty-four patients with AMI and hypertension in cardiovascular department of Quzhou People's Hospital were recruited.According to the pulse pressure level,the patients were categorized into low pulse pressure group and high pulse pressure group.The patients with left ventricular ejection fraction(LVEF)less than 50%were included in the low LVEF group.The differences of parameters between groups were compared,and correlation analysis and regression analysis were performed.Results Compared with high pulse pressure group,low pulse pressure group had higher BNP level,lower LVEF level,fewer patients without signs of heart failure(Killip I),and the difference was statistically significant(P<0.05).There was a positive correlation between pulse pressure and LVEF(r=0.242,P=0.007),and a negative correlation between pulse pressure and BNP(r=-0.192,P=0.033).Lower pulse pressure and higher BNP were independent risk factors for lower LVEF(P<0.05).Conclusion Early pulse pressure reduction has important clinical value in predicting secondary left ventricular dysfunction and heart failure in patients with AMI and hypertension.
