摘 要:目的分析2013年—2020年北京市门头沟区细菌性痢疾(简称菌痢)临床诊断病例感染病原谱,为该病的防控提供依据。方法收集2013年—2020年门头沟区肠道门诊菌痢临床诊断病例的粪便标本,对志贺菌属、沙门菌属、弯曲菌属、小肠结肠炎耶尔森菌、致泻性大肠埃希菌和副溶血性弧菌共6种常见致病菌进行分离培养、血清分型、分子分型及毒力基因检测。结果139份粪便标本检出志贺菌3株,其他细菌检出率从高到低依次为弯曲菌36.36%(12/33)、致泻性大肠埃希菌29.73%(11/37)、沙门菌27.03%(10/37)及副溶血性弧菌2.7%(1/37),未检出小肠结肠炎耶尔森菌。弯曲菌属中空肠弯曲菌检出率高于结肠弯曲菌。沙门菌包括肠炎、都柏林、乙型副伤寒等共5种血清型,以肠炎沙门菌为主。结论2013年—2020年北京市门头沟区菌痢临床诊断病例志贺菌检出率低,感染病原体呈多样性。Objective To analyze the pathogenic spectrum of clinically bacillary dysentery in Mentougou District,Beijing,2013-2020,and to provide evidence for prevention and control of the disease.Methods Fecal samples from clinical diagnosed bacterial dysentery cases in intestinal outpatient department of Mentougou District from 2013 to 2020 were collected.Six kinds of pathogenic bacteria including Shigella,Salmonella,Campylobacter,Yersinia enterocolitica,Escherichia coliand Vibrio parahaemolyticus were isolated and cultured.These bacterial isolates were further classified by serotyping,PFGE and real-time PCR.Results A total of 139 fecal specimens of clinical diagnosed bacterial dysentery cases were collected.3 strains of Shigella were isolated.The detection rate of other bacteria from high to low was following:Campylobacter 36.36%(12/33),Escherichia coli 29.73%(11/37),Salmonella 27.03%(10/37)and Vibrio parahaemolyticus 2.7%(1/37).Yersinia enterocolitica was not detected.The detection rate of Campylobacter jejuniwas higher than Campylobacter coli.Salmonella included 5 serotypes:S.enteritidis,S.dublin,S.paratyphi B,and the prevalence serotype was S.enteritidis.Conclusion The detection rate of Shigellain clinical diagnosed bacillary dysentery cases in Mentougou District of Beijing from 2013 to 2020 was low,and the infection pathogens were diverse.
