摘  要:目的探讨人巨细胞病毒(human cytomegalovirus,HCMV)感染对婴儿肝功能及免疫球蛋白水平的影响。方法选取2019年2月—2021年6月来郑州大学第三附属医院就诊的HCMV感染婴儿121例作为研究对象,HCMV-IgM抗体和HCMV DNA均阳性作为活动性感染组(n=57)、HCMV-IgM抗体阴性而HCMV DNA阳性作为潜伏性感染组(n=64),并选择同期来本院体检的健康婴儿59例作为对照组。比较分析3组婴儿的肝功能及免疫球蛋白水平。结果活动性感染组和潜伏性感染组丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、r-谷氨酰转移酶(GGT)、碱性磷酸酶(ALP)、总胆红素(TBIL)、直接胆红素(DBIL)及总胆汁酸(TBA)水平较对照组明显升高(P<0.05);活动性感染组ALT、AST、GGT、ALP、TBIL、DBIL及TBA水平较潜伏性感染组明显升高(P<0.05)。活动性感染组和潜伏性感染组IgG、IgA、IgM及IgE水平较对照组差异无统计学意义(P>0.05);活动性感染组IgG、IgA、IgM及IgE水平较潜伏性感染组差异无统计学意义(P>0.05)。结论HCMV感染可损害婴儿肝功能,但对婴儿的体液免疫无明显影响。Objective To analyze the effects of human cytomegalo virus(HCMV)on liver function and immunoglobulin levels in infants.Methods A total of 121 infants with HCMV were enrolled into this study in our hospital between February 2019 and May 2021.A total of 57 infants with positive HCMV-IgM and HCMV DNA were chosen as the active infection group.A total of 64 infants with positive HCMV DNA but negative HCMV-IgM were chosen as the latent infection group.At the same time,59 healthy infants were chosen as the control group.Results Compared with those of the control group,the levels of alamine transaminase(ALT),aspartate arninotransferase(AST),gamma-glu-tamyltransferase(GGT),alkaline phosphatase(ALP),total bilirubin(TBIL),direct bilirubin(DBIL)and total bile acid(TBA)were significantly higher in the latent infection group and the active infection group(P<0.05).Compared with those of the latent infection group,the levels of ALT,AST,GGT,ALP,TBIL,DBIL and TBA were significantly higher in the active infection group(P<0.05).Compared with those of the control group,the levels of IgG,IgA,IgM and IgE were not significantly different in the latent infection group and the active infection group,and the differences statistically significant(P>0.05).Compared with those of the latent infection group,the levels of immunoglobulin G(IgG),IgA,IgM and IgE were not different in the active infection group,and the differences statistically significant(P>0.05).Conclusion HCMV infection can cause fatal liver damage,but has no significant effect on humoral immunity in infants.
