温经活络外治法治疗多发性骨髓瘤合并早期周围神经病变临床研究

摘　　要：目的:观察温经活络外治法治疗多发性骨髓瘤合并早期周围神经病变的临床疗效。方法:将60例多发性骨髓瘤合并早期周围神经病变患者按照随机数字表法分为对照组和试验组,每组各30例。对照组给予常规对症治疗和营养保护,试验组在对照组治疗的基础上给予温经玉龙散外敷联合热奄包治疗。比较两组患者治疗前后周围神经病变分级和正中神经、尺神经、腓总神经、胫神经的运动神经传导速度(motor nerve conduction velocity,MCV)、感觉神经传导速度(sensory nerve conduction velocity,SCV)变化情况。结果:两组患者治疗后周围神经病变分级改善情况优于本组治疗前,且治疗后试验组优于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后正中神经、尺神经、腓总神经、胫神经的MCV、SVC大于本组治疗前,且治疗后试验组大于对照组,且差异具有统计学意义(P<0.05)。结论:温经活络外治法治疗多发性骨髓瘤合并早期周围神经病变,能有效延缓周围神经病变进展,缓解患者疼痛,提高生活质量。Objective:To observe the clinical curative effect of external therapy of warming meridians and activating collaterals in the treatment of multiple myeloma(MM)complicated with early peripheral neuropathy.Methods:A total of 60 patients with MM complicated with early peripheral neuropathy were randomly divided into the control group and the experimental group,with 30 cases in each group.The control group was given routine symptomatic treatment and nutritional protection.On the basis of the treatment of the control group,the experimental group was treated with external application of Meridian-Warming Yulong Powder(温经玉龙散,MWYLP)combined with hot compression bag.The grading of peripheral neuropathy and motor nerve conduction velocity(MCV)and sensory nerve conduction velocity(SCV)of median nerve,ulnar nerve,common peroneal nerve and tibial nerve were compared between the two groups before and after treatment.Results:The grading improvement of peripheral neuropathy in the two groups after treatment was better than that before treatment,and the improvement of the experimental group was better than that in the control group after treatment,the difference was statistically significant(P>0.05).After treatment,the MCV and SVC of median nerve,ulnar nerve,common peroneal nerve and tibial nerve in the two groups were higher than those before treatment,and the above indexes of the experimental group were higher than those of the control group after treatment,and all the differences were statistically significant(P<0.05).Conclusion:In terms of MM complicated with early peripheral neuropathy,external therapy of warming meridians and activating collaterals can effectively delay the progress of peripheral neuropathy,relieve patients′pain and improve patients′quality of life.
