速愈汤联合手术治疗肱骨中下段关节外骨折疗效观察

摘　　要：目的:观察速愈汤联合手术治疗肱骨中下段关节外骨折的临床疗效。方法:选取2019年6月至2021年6月兰考第一医院收治的118例肱骨中下段关节外骨折患者为研究对象,随机分为对照组和观察组各59例。两组均给予肱骨远端后外侧解剖锁定钢板固定术治疗,观察组术后3 d开始给予速愈汤,连续治疗6个月。比较两组患者治疗前后血清血栓素A2(thromboxane A2,TXA2)、I型胶原羧基端交联肽(carboxy-terminal telopeptide of typeⅠ,CTX-I)、护骨素(osteoprotegerin,OPG)水平,视觉模拟疼痛评分(visual analogy score,VAS)、骨密度情况,疼痛消失、骨折愈合时间和临床疗效。结果:观察组血清TXA2、CTX-I水平均低于对照组,OPG水平高于对照组,差异具有统计学意义(P<0.05)。观察组治疗后VAS评分均低于对照组,差异具有统计学意义(P<0.05)。观察组治疗后骨密度值高于对照组,差异具有统计学意义(P<0.05)。观察组疼痛消失及骨折愈合时间均短于对照组,差异具有统计学意义(P<0.05)。观察组有效率高于对照组,差异具有统计学意义(P<0.05)。结论:速愈汤联合手术治疗肱骨中下段关节外骨折,能够有效缓解患者疼痛,促进骨折愈合,提高临床疗效。Objective:To observe the clinical efficacy of Quick Cure Decoction combined with operation on extra-articular fracture of middle and lower humerus.Methods:A total of 118 patients with extra-articular fracture of the middle and lower humerus treated in Lankao First Hospital from June 2019 to June 2021 were randomly divided into the control group and the observation group,with 59 cases in each group.Both groups were treated with posterolateral anatomical locking plate fixation of distal humerus.The observation group was additionally treated with Quick Cure Decoction 3 days after operation.Both groups were treated for 6 months.The levels of serum thromboxane A2(TXA2),carboxy terminal peptide of type I collagen(CTX-I),osteoprotegerin(OPG),visual analogue score(VAS)and bone mineral density were compared between the two groups before and after treatment,and the disappearance of pain,fracture healing time and clinical efficacies were compared between the two groups.Results:After treatment,the levels of serum TXA2 and CTX-I in the observation group were lower than those in the control group,while the levels of OPG were higher than those in the control group,and all the differences were statistically significant(P<0.05).The VAS score of the observation group was lower than that of the control group after treatment,and the difference was statistically significant(P<0.05).The bone mineral density of the observation group was higher than that of the control group after treatment,and the difference was statistically significant(P<0.05).The time of pain disappearance and fracture healing in the observation group were shorter than those in the control group,and all the differences were statistically significant(P<0.05).The effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Operation combined with Quick Cure Decoction in the treatment of extra-articular fracture of the middle and lower humerus can effectively alleviate pain,promote
