摘 要:目的:考察2种耳用乳膏剂及其各药物成分对真菌性外耳道炎常见病原菌的抑菌活性。方法:根据医院制剂硼黄栓的处方药物构成,分别制备单方乳膏和2种复方乳膏,采用改良琼脂扩散法对所制备的乳膏进行4种标准菌株和2种临床分离菌株的体外抑菌实验,测定抑菌圈直径和最低抑菌浓度(MIC)。以醋酸曲安奈德益康唑乳膏作为对照,对2种复方乳膏进行抑菌效果比较。结果:硼酸、硼砂、盐酸苯海拉明单方乳膏对4种标准菌株均具有抑菌活性,雄黄和呋喃唑酮单方乳膏只对部分菌株有抑菌活性,冰片单方乳膏未显示抑菌活性。复方乳膏A和B对全部6种菌株均有抑菌效果。与对照乳膏相比,复方乳膏A和B对黑曲霉菌、白色念珠菌和近平滑念珠菌1的抑菌圈直径较小(P<0.01),对铜绿假单胞菌和近平滑念珠菌2的抑菌圈直径较大(P<0.01),对金黄色葡萄球菌的抑菌圈直径无明显差异。复方乳膏B对黑曲霉菌、白色念珠菌的抑菌圈直径大于复方乳膏A(P<0.05),对其余菌株无明显差异。结论:2种新型复方乳膏的抗真菌活性弱于对照药醋酸曲安奈德益康唑乳膏,但抑制细菌的活性优于对照药,预期其在治疗混合菌感染的外耳道炎方面具有一定优势。复方乳膏B的抑菌效果略优于复方乳膏A,且复方乳膏B处方更精简,制剂稳定性更好。OBJECTIVE To study the antimicrobial activity of two creams and their active components against common pathogens of fungal otitis externa.METHODS The active components of Penghuang suppository,a hospital preparation,were selected to prepare single component creams and compound creams.4 standard strains and 2 strains isolated from clinical samples were tested the antimicrobial activity of the cream by modified agar diffusion method in vitro.The diameter of inhibition circles and minimum inhibitory concentration(MIC)of each cream were recorded.The antimicrobial effects of the two compound creams were compared,using a commercial triamcinolone acetonide and econazole nitrate cream as control.RESULTS The single component cream composed of boric acid,borax or diphenhydramine hydrochloride had antimicrobial activity on all the 4 strains.Realgar and furazolidone cream only had antimicrobial activity on part of the strains,and borneol cream was not observed antimicrobial activity.Compound cream A and B had antimicrobial effects on all the 6 strains.Compared with the control,both of compound cream A and B had a smaller inhibitory circle on Aspergillus niger,Candida albicans and Candida parapsilosis 1(P<0.01),a larger inhibitory circle on Pseudomonas aeruginosa and Candida parapsilosis 2(P<0.01),and a similar inhibitory circle on Staphylococcus aureus.The inhibition circle of compound cream B on Aspergillus niger and Candida albicans was larger than that of compound cream A(P<0.05),and the inhibition circle on the other bacterial strains were similar for two creams.CONCLUSION The antifungal activity of the two compound creams was less active than the control,but the antibacterial activity is more effective.Compound creams are expected to have certain advantages in treating otitis externa with mixed microbial infections.Compound cream B have slightly better antimicrobial activity than cream A,and the prescription of compound cream B is more streamlined and the preparation stability is better.
