Taylor外固定架与单边外固定架治疗开放性胫腓骨骨折的比较研究

摘　　要：目的对比Taylor外固定架(Taylor spatial frame,TSF)与传统单边外固定架治疗开放性胫腓骨骨折的临床效果。方法2019年1月—2020年12月采用前瞻性研究的方法,将43例开放性胫腓骨骨折的患者按照完全随机化原则分为A组(22例,TSF固定组)和B组(21例,单边外固定架固定组)。比较2组患者的手术时间、术中出血量、外固定架拆除时间、骨折愈合时间、术后并发症的发生情况;术后分别对2组患者采用Johner-Wruhs评价标准评定患肢功能,对2组患者术后下肢力线进行评定。结果所有入组患者骨折均达到临床愈合标准,无不愈合、骨髓炎等并发症发生。TSF固定组手术时间、外固定架拆除时间及骨折愈合时间均少于单边外固定架固定组,差异有统计学意义(P<0.05)。患肢功能:TSF固定组优19例、良2例、可1例,优良率95.45%;单边外固定架组优15例、良4例、可2例,优良率90.48%,差异无统计学意义(P>0.05)。术后下肢力线:TSF固定组优20例、良1例、可1例,优良率95.45%;单边外固定架固定组:优19例,良1例、可1例,优良率95.24%,差异无统计学意义(P>0.05)。结论治疗胫腓骨开放性骨折Taylor外固定架组手术时间短、骨折愈合速度较快且术中创伤较小,减少了医患术中放射线的暴露时间,相对于单边外固定架组更具有优势。Objective To explore the clinical effects of Taylor spatial frame(TSF)and traditional unilateral external fixator in the treatment of open tibia and fibula fractures.Methods From January 2019 to December 2020,a prospective study was used to divide 43 patients with open tibia and fibula fractures into group A(22 cases,TSF fixation group)and group B(21 cases)according to the principle of complete randomization.Unilateral external fixing frame fixing group.There was no statistically significant difference between the two groups of patients in general information such as gender and age(P>0.05).The operation time,intraoperative blood loss,external frame removal time,fracture healing time,and occurrence of postoperative complications of group A and group B were compared respectively.After surgery,the two groups of patients were evaluated using the Johner-Wruhs evaluation standard.Results The operation time,external frame removal time and fracture healing time of the two groups A and B were less than the unilateral external fixation group,and the difference was statistically significant(P<0.05).The two groups of patients were followed up for 3 months after the external frame was removed,and the function of the affected limb was evaluated according to the Johner-Wruhs evaluation standard.TSF fixation group:19 cases were excellent,2 cases were good,1 case was fair,and the excellent and good rate was 95.45%;unilateral external fixation In the frame group,15 cases were excellent and good:4 cases,2 cases were fair,and the excellent and good rate was 90.48%;there was no significant difference in the excellent and good rate between the two groups(P>0.05).Six months after the operation,the lower limb force line of the two groups of patients was evaluated according to Luo Congfeng's evaluation standards.The TSF fixation group was excellent in 20 cases,good in 1 case,and fair in 1 case,with an excellent and good rate of 95.45%;unilateral external fixation group:19 cases were excellent,1 case was good,1 case was fair,and the excel
