蠲痹活络方预防奥沙利铂引起的周围神经毒性的临床观察

摘　　要：目的:研究外洗蠲痹活络方预防奥沙利铂引起的周围神经毒性的临床疗效。方法:从南京中医药大学江阴附属医院选取78例肿瘤化疗所致周围神经毒性患者,随机分为对照组和观察组,每组39例,对照组仅使用奥沙利铂化疗治疗,观察组采用蠲痹活络方外洗联合奥沙利铂化疗的治疗方案,化疗后从第1天连续外洗3周。记录两组患者化疗前后血清NGF水平、化疗后周围神经毒性程度和发生率以及化疗前后KPS评分。结果:化疗前两组血清NGF水平无统计学意义(P>0.05),化疗后观察组血清NGF水平显著高于对照组(P<0.05);化疗后观察组周围神经毒性发生程度及发生率显著低于对照组(P<0.05);化疗前两组KPS评分无统计学意义(P>0.05),化疗后观察组KPS评分显著高于对照组(P<0.05);化疗前两组感觉神经和运动神经的传导速度无统计学意义(P>0.05),化疗后观察组传导速度减慢程度显著低于对照组(P<0.05)。结论:患者化疗后通过外洗蠲痹活络方能够降低周围神经毒性发生程度和发生率,提高患者生活质量。Objective: To study the clinical efficacy of external application of Juanbi Huoluo Formula in preventing peripheral neurotoxicity caused by Oxaliplatin. Methods: 78 patients with peripheral neurotoxicity caused by tumor chemotherapy were randomly divided into the control group and the study group, with 39 cases in each group. The control group was treated with Oxaliplatin chemotherapy only;on which basis, the study group was also treated with external washing of Juanbi Huoluo Formula, which was applied for three consecutive weeks from the first day after chemotherapy. The serum level of NGF, the degree and incidence of peripheral neurotoxicity, as well as KPS score were observed before and after chemotherapy in the two groups. Results: There was no statistical difference in the serum level of NGF between the two groups before chemotherapy(P>0.05), which was significantly higher in the study group than that in the control group after chemotherapy(P<0.05). The degree and incidence of peripheral neurotoxicity were significantly lower in the study group than those in the control group after chemotherapy(P<0.05). There was no statistical difference in KPS scores between the two groups before chemotherapy(P>0.05), which was significantly higher in the study group than that in the control group after chemotherapy(P<0.05). There was no statistical difference in conduction velocity of sensory nerves and motor nerves between the two groups before chemotherapy(P>0.05);which was significantly slower in the study group than that in the control group after chemotherapy(P<0.05). Conclusion: After chemotherapy, external washing of Juanbi Huoluo Formula can reduce the degree and incidence of peripheral neurotoxicity and improve the life quality of the patients.
