基于结构式家庭治疗的随访管理对老年永久性肠造口患者生活质量及心理社会适应能力的影响

摘　　要：目的:探讨基于结构式家庭治疗的随访管理对老年永久性肠造口患者生活质量及心理社会适应能力的影响。方法:将2018年6月至2020年6月于我院接受永久性肠造口治疗的86例老年患者随机分为观察组和对照组,各43例。对照组接受常规出院指导及随访管理,观察组接受基于结构式家庭治疗的随访管理。比较2组干预前、干预3个月后患者的生活质量及心理社会适应能力。生活质量采用中文版造口病人生活质量量表(Stoma-QOL)评价,心理社会适应能力采用中文版造口患者社会心理适应量表(OAI-20)评价。结果:干预3个月后,观察组患者Stoma-QOL之身心状况、社会交往、人际关系、造口袋对患者的影响维度评分及总分均明显高于对照组,而且OAI-20之正性情绪、负性情绪、社会生活适应维度评分及总分均明显高于对照组,P<0.05。结论:对老年永久性肠造口患者实施基于结构式家庭治疗的随访管理,有助于提高其心理社会适应能力,进而改善生活质量。Objective To invesigate the influence of follow-up management based on structural family treatment on the quality of life and mental social adaptation ability of senior patients with permanent bowel stoma.Methods Randomly divided 86senior patients who received permanent bowel ostomy in author’s hospital(2018-06-2020-06)into control group(43cases,routine discharged direction,and follow-up management)and observation group(43cases,the follow-up management based on structural family treatment),compared both groups’patients’quality of life[evaluating by using stoma-QOL(Chinese edition)],and mental social adaptation ability[evaluating by using OAI-20(Chinese edition)].Results Intervention3months later,according to stoma-QOL,in the score ratings on patients’psychosomatic situation,social association,interpersonal relationships,as well as stoma-bag’s influence dimension on patients,and in the total points of above items observation group was significantly higher than control group(P<0.05);meanwhile,according to OAI-20,in the score ratings on positive mood,negative moon,and social life adaptation dimensions,and in the total points of above items,observation group was significantly higher than control group(P<0.05).Conclusion For senior patients with permanent bowel stoma implementing follow-up management based on structural family treatment is helpful to enhance their mental social adaptation ability,and to improve their quality of life.
