摘  要:目的基于网络药理学方法探究中药丹参及其活性单体成分在治疗冠心病合并抑郁和(或)焦虑障碍患者中的潜在作用机制。方法连续收集2021年3月至4月间我院心内科收治的147例冠心病患者的临床资料,包括常规生化检查和用药情况,调查中药复方制剂在冠心病治疗中的使用情况;基于TCMSP、PubChem及SwissTargetPrediction数据库筛选丹参活性单体的作用靶点,利用OMIM、GeneCards及TTD疾病数据库,挖掘抑郁焦虑的相关基因。匹配中药与疾病靶点,进行蛋白互作网络分析和GO、KEGG基因富集分析,利用CytoScape软件进行中药-靶点-疾病网络分析。结果147例冠心病患者治疗中最常用的中药制剂是舒血宁(44.9%)、丹参(27.9%)及大株红景天(21.8%),不同性别间使用频次无明显差异。网络药理学分析显示,丹参活性成分可能作用于神经活性配体-受体结合、5-羟色胺能突触、多巴胺能突触等,参与调节神经递质转运、多巴胺与儿茶酚胺类代谢及应激反应等生物学过程。中药-靶点-疾病网络分析显示,丹参多种活性成分与儿茶酚-O-甲基转移酶、单胺氧化酶、5-羟色胺受体及阿片类受体等存在相互作用,可能成为丹参改善抑郁和焦虑的潜在作用靶点。结论丹参参与调节神经递质与突触活动,有望成为临床治疗冠心病合并抑郁或焦虑障碍的潜在药物。Objective To explore the potential mechanism of Salvia miltiorrhiza and its active monomers in patients with coronary heart disease(CHD)complicated with depression or anxiety disorder based on network pharmacology.Methods We collected the clinical data of 147 patients with CHD admitted to our hospital from March to April 2021 and investigated biochemical examination,drug treatment and the use of Chinese herbal compound.The downstream targets of active monomers of Salvia miltiorrhiza were screened based on TCMSP,PubChem and SwissTargetPrediction databases,and depression or anxiety-related genes were mining in OMIM,GeneCards and TTD databases.Protein-protein interaction network and GO and KEGG gene enrichment analyses were performed by matching Chinese herbal compound with disease targets.Chinese herbal compound-target-disease network was built by Cytoscape.Results Shuxuening(44.9%),Salviae miltiorrhiza(27.9%),and Sofren injection(21.8%)were the most commonly used Chinese herbal or injections in the clinical treatment of CHD,and there was no difference in the frequency of use between genders.Network pharmacology showed that the active components of Salvia miltiorrhiza might act on neuroactive ligand-receptor binding,5-hydroxytryptamine synapses and dopaminergic synapses and participate in the regulation of neurotransmitter transport,dopamine and catecholamine metabolism and stress response.Target-disease network analysis showed that many active components of Salvia miltiorrhiza interacted with catechol-O-methyltransferase,monoamine oxidase,5-hydroxytryptamine receptor and opioid receptor,which may be the potential targets of Salvia miltiorrhiza in improving depression and anxiety.Conclusion Salvia miltiorrhiza is involved in the regulation of neurotransmitters and synaptic activities,thus making it a potential drug for the clinical treatment of CHD complicated with depression and anxiety disorder.
