长沙市某三甲医院院内急诊就诊患者疾病谱特征分析

摘　　要：目的收集2013年1月1日—2018年12月30日长沙市某三甲医院医院急诊医疗系统共319736例患者就诊信息,对就诊信息进行分析,为该医院和长沙市医院的应急管理提供依据。方法采用大数据技术方法,对长沙市某三甲医院急诊科就诊信息系统6年内319736例患者的病历进行描述和统计分析,包括患者的性别、年龄和疾病类型。结果6年来,在医院急诊就诊疾病中,前三位为消化系统急诊(85876)例26.85%、呼吸系统急诊(66468)例20.78%、常见损伤(31627)例17.66%,消化系统疾病是最高的疾病谱,均呈快速增长趋势,夏季是院内急救的高发期,大多数20~39岁患者,男性多于女性,男性约是女性的1.08倍,1 d内就诊有两个高峰期,最高的高峰期是20:00~21:00。结论①6年来,医院的急诊就诊人数逐年增加,因此,有必要增加医务人员的数量,加强对医务人员的培训,以应对今后就诊人数的增加。②在6年内,除了男性的性别特征、社会角色外,男性比女性多,而且与城市中男性和女性的比例也有关。③所有系统性疾病中,消化系统急症居院内急诊首位,这可能与当地的饮食习惯密切相关。因此,如何减少消化系统突发事件是长沙市面临的一个重要健康问题。④该医院急诊季节规律性明显,夏季是高发季节,根据疾病谱的季节特点,合理调整院外和院内资源,提前加强相关技能培训。Objective From January 1,2013 to December 30,2018,there were 319736 patients in the hospital emergency medical system of a third Class Hospital in Changsha City.Analysis of the information,to provide the basis for the hospital and Changsha hospital emergency management.Methods By using the method of big data technology,the medical records of 319736 patients in the first aid system of a third Class.A Hospital in Changsha City within 6 years were described and analyzed statistically,including the sex,age and disease type of the patients.Results For six years,in the hospital emergency medical treatment.The top three emergency cases is digestive system 85876(26.85%),emergency respiratory system(66468)20.78%,common injuries(31627)17.66%,digestive system disease is the highest disease spectrum,all showing a rapid growth trend,summer is the high incidence of hospital first aid,most 20~39 years old patients,men more than women,men are about 1.08 times as many as women,there are two peak periods in a day.The highest peak is 20:00~21:00.Conclusion①In the past six years,the number of first aid in the hospital has increased year by year,so it is necessary to increase the number of medical staff and strengthen the training of medical staff to cope with the increase in the number of visits in the future.②Within six years,there were more men than women,in addition to male gender characteristics social roles,and is also relevant with the proportion of men and women in the city.③Among all systemic diseases,digestive system emergencies are the first in hospital all year round,which may be closely related to local eating habits.Therefore,how to reduce digestive system emergencies is an important health problem facing the city.④The seasonal regularity of emergency department in this hospital is obvious,summer is the season of high incidence.According to the seasonal characteristics of disease spectrum,the resources in front of hospital and hospital should be adjusted reasonably,and the relevant skills training should be stre
