摘  要:目的观察护骨胶囊灌服对激素性股骨头坏死(ONFH)大鼠股骨头坏死的改善作用,并分析其作用机制。方法取SD大鼠56只,随机分成空白对照组4只和模型组52只。空白对照组正常饮食,模型组给予脂多糖和甲强龙联合注射诱导制作ONFH模型。造模完成后,随机取4只处死,观察股骨头形态学变化,以出现骨坏死、骨小梁断裂和骨髓细胞脂肪化为造模成功。将模型组剩余48只大鼠分为模型对照组、护骨胶囊基础剂量组和护骨胶囊双倍剂量组各16只,护骨胶囊基础剂量组给予护骨胶囊0.486 g/(kg·d)灌胃,双倍剂量组给予护骨胶囊0.972 g/(kg·d)灌胃,模型对照组给予10 mL/(kg·d)生理盐水灌胃,每天1次。空白对照组于药物干预前全部处死,其他三组分别于干预42 d和60 d后各取8只大鼠处死,心脏取血,检测血清骨形成标志物骨特异性碱性磷酸酶(BALP)、Ⅰ型前胶原氨基末端肽(PINP)和骨吸收标志物抗酒石酸酸性磷酸酶(TRACP5b)、Ⅰ型胶原交联C-末端肽(CTX)水平;取右股骨头,脱钙、切片,采用甲苯胺蓝染色,观察股骨头形态学变化。结果与空白对照组相比,模型对照组干预42 d和60 d后血清BALP、TRAP5b、PINP、CTX浓度均升高(P均<0.01)。与模型对照组相比,护骨胶囊基础剂量组和双倍剂量组干预42 d和60 d后,血清BALP、TRAP5b、PINP、CTX均降低(P均<0.01),其中双倍剂量组上述指标均较基础剂量组降低(P均<0.01);在相同剂量组内,干预42 d与60 d相比差异无统计学意义(P均>0.05)。与空白对照组相比,模型对照组出现股骨头坏死特征,包括关节软骨结构紊乱,骨小梁断裂、坏死和吸收成碎片,骨髓造血细胞缺失,脂肪细胞融合成空泡状;软骨下骨出现淤血,骺板处软骨细胞柱出现水平断裂。与模型对照组相比,护骨胶囊基础剂量组和护骨胶囊双倍剂量组组均表现出骨坏死减轻的特征,如关节软骨结构趋向正常,软骨下区骨形成增强,Objective To investigate the effects of Hugu capsules on the osteonecrosis of femoral head(ONFH)of steroid-induced ONFH rats.Methods Fifty-six SD rats were randomly divided into the control group(4 rats)and model group(52 rats).The rats in the blank control group had normal diet,and the ONFH models were made in the model group by combined injection of lipopolysaccharide and methylprednisolone.After the completion of modeling,4 randomly selected rats were killed to observe the morphological changes of femoral head.The occurrence of bone necrosis,bone trabecular fracture and bone marrow cell steatosis indicated success of modeling.The remaining 48 rats of the model group were divided into the model control group,basic-dose Hugu capsule group,and double-dose Hugu capsule group,with 16 rats in each group.The rats in the basic-dose Hugu capsule group were given 0.486 g/(kg·d)Hugu capsule by gavage,the rats in the double-dose group were given 0.972 g/(kg·d)Hugu capsule by gavage,and the rats of the model control group were given 10 mL/(kg·d)normal saline by gavage,once a day.All rats in the blank control group were killed before the drug intervention,and 8 rats from each group of the other four groups were killed at 42 and 60 days after the intervention,respectively.Blood was taken from the heart to detect the levels of serum bone formation markers bone specific alkaline phosphatase(BALP),amino-terminal propeptide of typeⅠcollagen(PINP),bone resorption markers tartrate-resistant acid phosphatase type 5b(TRACP5b)and cross-linked C-telopeptide of type Ⅰ collagen(CTX).The right femoral head was taken,decalcified and sliced,and stained with toluidine blue to observe the morphological changes of the femoral head.Results Compared with the blank control group,the concentrations of serum BALP,TRAP5b,PINP and CTX in the model control group increased at 42 and 60 days after intervention(all P<0.01).Compared with the model control group,after 42 and 60 days of intervention,the serum BALP,TRAP5b,PINP and CTX in the basic-do
