中药熏浴联合PNF治疗ACLR术后膝关节功能障碍的效果研究

摘　　要：目的:观察中药熏浴联合神经肌肉本体感觉促进技术(proprioceptive neuromuscular facilitation,PNF)治疗前交叉韧带重建(anterior cruciate ligament reconstruction,ACLR)术后膝关节功能障碍的临床效果。方法:ACLR术后膝关节功能障碍患者共48例,分为两组,各24例,均予常规康复及PNF治疗,观察组另增加中药熏浴疗法。比较两组患者治疗前、治疗4周后疼痛视觉模糊评分(visual analogue scale/score,VAS)、肿胀评分、关节主动活动度(active rang of motion,AROM)、位置觉测定、Berg评分(Berg balance scale,BBS)以及膝关节功能(Lysholm knee score/scale,LKSS)评分。结果:两组患者治疗前VAS、肿胀评分、AROM、位置觉、BBS、LKSS比较差异均无统计学意义(均P>0.05);治疗4周后,观察组6项评分均优于对照组(均P<0.05)。结论:与单纯采用PNF相比,中药熏浴疗法联合PNF的康复治疗方法对ACLR术后膝关节功能障碍的临床疗效更为显著。Objective:To observe the clinical effect of traditional Chinese medicine fumigation bath combined with proprioceptive neuromuscular facilitation(PNF)in the treatment of postoperative knee dysfunction after anterior cruciate ligament reconstruction(ACLR).Methods:48 patients with knee dysfunction after ACLR were divided into observation group and control group,with 24 cases in each group.The two groups were given routine rehabilitation and PNF treatment,and the observation group was added traditional Chinese medicine fumigation bath therapy.Visual analogue scale/score(VAS),swelling,active rang of motion(AROM),position perception,Berg balance scale(BBS)and Lysholm knee score/scale(LKSS)were scored in two groups before treatment,and 4 weeks after treatment,respectively,and a comparative analysis was made.Results:There was no significant difference in VAS,swelling,AROM,position sense,BBS and LKSS between two groups before treatment(all P>0.05).After 4 weeks of treatment,the 6 indicators in the observation group were better than those in the control group(all P<0.05).Conclusion:Compared with PNF alone,traditional Chinese medicine fumigation bath combined with PNF has a more significant clinical effect on postoperative knee dysfunction after ACLR.
