摘  要:目的分析广州市2017—2020年流感流行特征。方法通过EXCEL建立数据库整理分析中国疾病预防控制信息系统中广州市流感监测流感样病例(influenza-like illness,ILI)数据、病原学监测数据、报告病例数等信息。结果2017—2020年广州市累计报告流感确诊病例(含临床诊断病例,按发病日期统计)230563例,累计监测ILI样本16862例,阳性样本2035例,主要分布于3—7月以及11月至次年1月;流感病例主要为<10岁婴幼儿及儿童;男性病例数较女性高;职业分布主要为学生,77805例(33.75%)、散居儿童64359例(27.91%)和幼托儿童36839例(15.98%);2017—2020年报告发病率最高的区分别为从化区(398.77/万)、南沙区(568.74/万)、天河区(1451.51/万)、从化区(483.76/万);病原学监测显示,历年检出流感病毒优势毒株交替流行:2017年主要流行的优势毒株为A(H3N2)型,2018年优势毒株为B(Yamagata)型,2019年主要检测毒株为A(H1N1)pdm09型,2020年主要为A(H3N2)型;40~<50岁组、10~<20岁组检出率最高,分别为14.34%和13.90%,广州市红十字会医院整体检出率最高(14.31%)。结论广州市流感流行季为3—7月以及11月至次年1月,A(H3N2)型、A(H1N1)pdm09型、B(Yamagata)型流感病毒交替作为优势毒株流行;新型冠状病毒肺炎疫情防控措施对流感防控有积极正面的影响。Objective To understand the epidemiological characteristics of influenza in Guangzhou from 2017 to 2020.Methods Influenza-like illness monitoring(ILI)data,pathogen surveillance data,reported case number and other related information in Guangzhou were acquired from China Disease Prevention and Control Information System and were processed and analyzed using EXCEL.Results A total of 230563 confirmed cases with influenza were reported from 2017 to 2020 in Guangzhou,16862 cases with ILI were monitored and 2035 were tested positive,which were mainly found from March to July,and November to January.Majority of cases were in the age group of<10,and there were more male cases than female ones(male:female=1.19∶1).Most cases were students(33.75%),scattered children(27.91%)and preschooler(15.98%).The highest incidence rates(1/100000)were reported in Conghua district(398.77),Nansha district(568.74),Tianhe District(1451.51)and Conghua District(483.76)during 2017 and 2020.Pathogen surveillance analysis showed that the dominant influenza virus varied with A(H3 N2)in 2017,B(Yamagata)in 2018,A(H1 N1)pdm 09 in 2019 and again A(H3 N2)in 2020.The positive rates were the highest in 40-<50 year-old group(14.34%)and 10-<20 year-old age(13.90%).Guangzhou Red Cross Hospital had the highest overall positive rate of 14.31%.Conclusions Influenza virus strains A(H3 N2),A(H1 N1)pdm09 and B(Yamagata)alternately circulate as the dominant strains during the influenza season in Guangzhou from March to July and from November to January of the following year.COVID-19 preventive strategies can positively impact the prevention and control of influenza.
