三联抗菌新药Recarbrio治疗复杂腹腔感染的研究进展

摘　　要：复杂腹腔感染(complicated intra-abdominal infections, cIAI)是临床上一种发病率高和死亡率高的腹腔感染病症,术后抗生素治疗对cIAI患者的恢复起到至关重要的作用。为了解决cIAI病原菌对传统碳青霉素类抗生素的耐药性,在β-内酰胺类抗生素中加入β-内酰胺酶抑制剂,可以恢复β-内酰胺类抗生素对绝大多数耐药革兰阴性、阳性菌的抗菌活性。2019年7月16日,美国FDA批准一种用于治疗成人cIAI的三联抗菌新药Recarbrio(亚胺培南、西司他丁钠和雷巴坦)。该三联抗菌药是在原有的亚胺培南/西司他丁钠基础上,添加一种新型的β-内酰胺酶抑制剂“雷巴坦”,旨在解决亚胺培南耐药率上升的问题。本文主要就cIAI有关研究现状、三联新药Recarbrio以及Recarbrio治疗cIAI研究进展三方面进行综述。Complicated intra-abdominal infections(cIAI) is a clinical abdominal infection with high morbidity and mortality. Postoperative antibiotic therapy plays a key role in the recovery of cIAI patients. In order to resolve the resistance of cIAI pathogens to traditional carbapenem antibiotics, a β-lactamase inhibitor is added to β-lactam antibiotics to restore the antibacterial activity of the latter against most resistant Gram-negative and positive bacteria. A new triple antibacterial drug Recarbrio(imipenem, cilastatin sodium and relebactam) was approved by the U.S. Food and Drug Administration(FDA) for the treatment of adult cIAI on July 16 2019. The triple antibiotic was obtained by adding a new β-lactamase inhibitor “relebactam” to original imipenem and cilastatin sodium, in order to address the rising rate of imipenem resistance. The current research status of cIAI, the new triple drug Recarbrio and related treatment progress of cIAI by Recarbrio are reviewed in this article.
