雷公藤的临床应用及其用量探究

摘　　要：通过搜集古医籍及现代医家临床经验,总结出雷公藤有以下特点:在汤剂中用量为1~50 g,多为10~15 g,外用药中多为1.85~11.25 g。临床根据具体疾病及结合中药配伍关系,寻求最佳用量,如常配伍威灵仙、独活、秦艽等,治疗免疫系统疾病(类风湿性关节炎、强直性脊柱炎、系统性红斑狼疮等),汤剂为5~50 g,外用常为1.85~11.25 g;常配伍水牛角、牡丹皮等,治疗肾系疾病(IgA肾病、慢性肾小球肾炎、肾病综合征、糖尿病肾病等),汤剂中用量为6~15 g;常配伍白花蛇舌草、半边莲等,治疗多种癌症术后病症(肺癌、乳腺癌等),汤剂中用量为5~6 g;常配伍夏枯草、鸡血藤等,治疗甲状腺疾病(桥本甲状腺炎),汤剂中用量为15~45 g。Through the collection of ancient medical books and the clinical experience of modern physicians,it is concluded that Tripterygium wilfordii has the following characteristics:Its dosage in clinical decoction is 1-50 g,while usually 10-15 g,and in external medicines,1.85-11.25 g.According to specific diseases and its combination with various medicinal herbs,its optimal dosage is selected.For instance,its dosage usually ranges from 5 to 50 g in decoction,while from 1.85 to 11.25 g for external use when Tripterygium wilfordii is combined with Chinese clematis root,doubleteeth pubescent angelica root,largeleaf gentian root,etc.,to treat immune system diseases(rheumatoid arthritis,ankylosing spondylitis,systemic lupus erythematosus,etc.).It is often used in combination with buffalo horn and tree peony root bark for the treatment of renal diseases(IgA nephropathy,chronic glomerulonephritis,nephrotic syndrome,diabetic nephropathy,etc.)with a dosage range of 6-15 g in decoction.It is often used in combination with hedyotis,Chinese lobelia herb,etc.to treat a variety of cancer-related postoperative symptoms(lung cancer,breast cancer,etc.)with its dosage ranging from 5 to 6 g in decoction.And it is often compatible with common selfheal fruit-spike,suberect spatholobus stem,etc.,for the treatment of thyroid diseases(Hashimoto’s thyroiditis)with a dosage range of 15-45 g in decoction.
