冻干重组人脑利钠肽联合左西孟旦治疗老年急性左心衰竭患者的效果及对BNP水平的影响

摘　　要：目的探究冻干重组人脑利钠肽联合左西孟旦治疗老年急性左心衰竭患者的效果及对BNP水平的影响。方法回顾性分析2018年10月至2020年12月本院收治的90例老年急性左心衰竭患者的临床资料,根据治疗方式不同将其分为参照组(n=45)与研究组(n=45)。参照组开展冻干重组人脑利钠肽治疗,研究组开展冻干重组人脑利钠肽联合左西孟旦治疗。比较两组患者的临床疗效、不良反应发生情况、肾功能、心血管功能、血气指标、血清炎性因子水平。结果研究组的治疗总有效率高于参照组,差异具有统计学意义(P<0.05)。研究组的不良反应总发生率与参照组比较,差异不具有统计学意义(P>0.05)。治疗后,两组患者24 h尿量均增多,肾小球滤过率均升高,Scr水平均下降,且研究组优于参照组,差异具有统计学意义(P<0.05)。治疗后,两组患者BNP水平较治疗前均下降,LVED、LVEF、SV较治疗前均上升,且研究组优于参照组,差异具有统计学意义(P<0.05)。治疗后,两组患者PaO_(2)、PaO_(2)/FiO_(2)、pH较治疗前均上升,PaCO_(2)较治疗前均下降,且研究组优于参照组,差异具有统计学意义(P<0.05)。治疗后,两组患者IL-8、IL-6、TNF-α、hs-CRP水平较治疗前均下降,且研究组低于参照组,差异具有统计学意义(P<0.05)。结论冻干重组人脑利钠肽联合左西孟旦治疗老年急性左心衰竭患者可提升疗效,改善肾功能、心血管功能、血气指标、血清炎性因子水平,效果显著。Objective To explore the effect of lyophilized recombinant human brain natriuretic peptide combined with levosimendan in the treatment of elderly patients with acute left heart failure and its influence on BNP level.Methods Clinical data of 90 elderly patients with acute left heart failure treated in our hospital from October 2018 to December 2020 were analyzed retrospectively,according to different treatment methods,the patients were divided into reference group(n=45)and study group(n=45).The reference group was treated with lyophilized recombinant human brain natriuretic peptide,and the study group was treated with lyophilized recombinant human brain natriuretic peptide combined with levosimendan.The clinical efficacy,occurrence of adverse reactions,renal function,cardiovascular function,blood gas indexes and serum inflammatory factors levels were compared between the two groups.Results The total effective rate of treatment in the study group was higher than that in the reference group,and the difference was statistically significant(P<0.05).There was no statistically difference in the total incidence of adverse reactions between the study group and the reference group(P>0.05).After treatment,the 24 h urine volume and the glomerular filtration rate increased,the Scr level decreased in both groups,and those in the study group were better than the reference group,the differences were statistically significant(P<0.05).After treatment,BNP level decreased and LVED,LVEF and SV increased compared with those before treatment in both groups,and those in the study group were better than the reference group,the differences were statistically significant(P<0.05).After treatment,PaO_(2),PaO_(2)/FiO_(2) and pH increased and PaCO_(2) decreased in both groups compared with those before treatment,and those in the study group were better than the reference group,the differences were statistically significant(P<0.05).After treatment,the levels of IL-8,IL-6,TNF-αand hs-CRP decreased compared with those before treatment in both gr
