摘　　要：目的探讨电针预处理对全髋关节置换术后老年患者疼痛及免疫功能的影响。方法选取2020年1月至2021年3月于我院行全髋关节置换术的74例老年患者作为研究对象,采用随机数字表法将其分为试验组(n=37)与对照组(n=37)。试验组接受电针预处理,对照组不接受电针预处理。比较两组的VAS评分、T淋巴细胞亚群指标及血清学指标。结果术后6、12、24 h,试验组的VAS评分低于对照组,差异具有统计学意义(P<0.05)。术后24 h,试验组的CD3^(+)、CD4^(+)水平及CD4^(+)/CD8^(+)高于对照组,CD8^(+)水平低于对照组,差异具有统计学意义(P<0.05)。术后24 h,试验组的IL-1β、TNF-α、ACTH水平低于对照组,差异具有统计学意义(P<0.05)。结论电针预处理可抑制全髋关节置换术老年患者围术期的炎症反应及应激反应,减轻术后疼痛程度,改善机体免疫功能。Objective To investigate the effects of electroacupuncture preconditioning on pain and immune function in elderly patients after total hip arthroplasty.Methods Seventy-four elderly patients who underwent total hip arthroplasty in our hospital from January 2020 to March 2021 were selected as the study objects and divided into experimental group(n=37)and control group(n=37)by random number table method.The experimental group received electroacupuncture preconditioning,while the control group did not receive electroacupuncture preconditioning.The VAS score,T lymphocyte subsets and serological indexes were compared between the two groups.Results At 6,12 and 24 h after operation,the VAS score of the experimental group was lower than that of the control group,and the difference was statistically significant(P<0.05).At 24 h after operation,the levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the experimental group were higher than those in the control group,the level of CD8^(+)was lower than that in the control group,and the differences were statistically significant(P<0.05).At 24 h after operation,the levels of IL-1β,TNF-α and ACTH in the experimental group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Electroacupuncture preconditioning can inhibit the perioperative inflammatory response and stress response,reduce the degree of postoperative pain and improve the immune function of elderly patients undergoing total hip arthroplasty.
