血清TGAb、TMAb及TPOAb水平在自身免疫性甲状腺病诊断中的价值

摘　　要：目的:探讨血清甲状腺球蛋白抗体(TGAb)、甲状腺微粒体抗体(TMAb)、甲状腺过氧化物酶抗体(TPOAb)在自身免疫性甲状腺病中的诊断价值。方法:选取2019年1月-2020年1月某院收治的169例自身免疫性甲状腺病患者作为观察组,选取同期85例健康体检者作为对照组,检测两组血清TGAb、TMAb及TPOAb水平并对其结果进行分析。结果:研究组甲状腺功能亢进、甲状腺功能减退、单纯甲状腺肿大患者的TGAb、TMAb、TPOAb阳性率、检出水平均高于对照组(P<0.05);血清TGAb、TMAb、TPOAb与甲状腺功能亢进呈负相关(r=-4.525、-0.521、-0.422,P<0.05),与甲状腺功能减退、单纯甲状腺肿大呈正相关(r=0.518、0.680、0.469、0.529、0.475、0.614,P<0.05)。结论:自身免疫性甲状腺病患者血清TGAb、TMAb、TPOAb水平明显升高,并和疾病密切相关,临床中对血清甲状腺自身抗体进行检测有利于确诊甲状腺疾病类型给予有效治疗。Objective:To explore the diagnostic value of serum thyroglobulin antibody(TGAb),thyroid microsomal antibody(TMAb)and thyroid peroxidase antibody(TPOAb)in the diagnosis of autoimmune thyroid disease.Methods:A total of 169 patients with autoimmune thyroid disease who were admitted to the hospital from January 2019 to January 2020 were enrolled as observation group,while another 85 health examinations during the same period were enrolled as control group.The levels of serum TGAb,TMAb and TPOAb in both groups were detected,and the results were analyzed.Results:The positive rates and detection levels of TGAb,TMAb and TPOAb in patients with hyperthyroidism,hypothyroidism and simple goiter in study group were higher than those in control group(P<0.05).The serum TGAb,TMAb and TPOAb were negatively correlated with hyperthyroidism(r=-4.525,-0.521,-0.422,P<0.05),while positively correlated with hypothyroidism and simple goiter(r=0.518,0.680,0.469,0.529,0.475,0.614,P<0.05).Conclusion:The levels of serum TGAb,TMAb and TPOAb are significantly increased in patients with autoimmune thyroid disease,which are closely related to the disease.The clinical detection of serum thyroid autoantibodies is beneficial to determine the types of thyroid disease and give effective treatment.
