常态化肺保护对老年慢阻肺患者肺通换气功能、炎症反应及胸肺顺应性的影响研究

摘　　要：目的:研究常态化肺保护的实施方法及在老年COPD患者护理中的应用价值。方法:将某院收治的120例老年COPD患者随机分为C组和Y组各60例,C组采用常规护理,Y组采用常态化肺保护护理。于护理干预前后,分别检测肺通换气功能指标[气道阻力(Raw)、每分钟最大通气量(MVV)、最大呼气中段流量(MMF)]、炎症反应指标[血清降钙素原(PCT)、超敏C-反应蛋白(hs-CRP)和肿瘤坏死因子-α(TNF-α)]和胸肺顺应性指标[肺顺应性(CL)、胸廓顺应性(CTH)、总顺应性(CT)]。结果:护理后Y组患者Raw低于C组(P<0.05),MVV、MMF均高于C组(P<0.05),Y组患者血清hs-CRP、PCT、TNF-α均低于C组(P<0.05),Y组患者CL、CTH、CT均高于C组(P<0.05)。结论:通过予以老年COPD患者常态化肺保护护理能有效改善患者肺通换气功能,提高炎症反应的抑制效果,提高胸肺顺应性。Objective:To study the implementation method of normalized lung protection and its application value in the nursing elderly patients with COPD.Methods:120 cases of elderly COPD patients admitted to a hospital were randomly divided into group C(60 cases)and group Y(60 cases),and the two gorups were given routine nursing and normalized lung protection nursing,respectively.Before and after nursing intervention,the lung ventilation function indexes Raw,maximum ventilation volume per minute(MVV)and maximum mid-expiratory flow(MMF),inflammatory response indexes serum procalcitonin(PCT),hypersensitive C-reactive protein(hs-CRP)and tumor necrosis factor-α(TNF-α),and thoracopulmonary compliance indexes lung compliance(CL),thoracic compliance(CTH),total compliance(CT)were detected.Results:After nursing,Raw in group Y was lower than that in group C(P<0.05),MVV and MMF were higher than those in group C(P<0.05),serum hs-CRP,PCT and TNF-αin group Y were lower than those in group C(P<0.05),and CL,CTH and CT in group Y were higher than those in group C(P<0.05).Conclusion:Normalized lung protection nursing for elderly COPD patients can effectively improve lung ventilation function,improve the inhibition effect of inflammatory response,and improve the thoracopulmonary compliance.
