基于“肝藏魂”治疗多梦症思路探析

摘　　要：多梦症是以睡眠周期内反复出现恐怖不安或焦虑的梦境体验为特征的睡眠障碍类疾病。基于“肝藏魂”理论探讨多梦症病因病机和综合疗法的辨证论治思路。中医学认为多梦症是肝脏受邪,肝不藏魂,魂不守舍,肝魂妄动所致。基于“肝藏魂”理论治疗多梦症采用情志疗法和药物治疗相结合的综合疗法,以达到身心同治的目的。其中情志疗法包括移精变气法、从意共情法、情志相胜法、惊者平之法。药物治疗从肝论治,水不涵木、肝魂妄动证应用滋水涵木、养阴安魂法;神虚胆怯、肝失藏魂证应用安神定志、收摄肝魂法;痰火内扰、肝魂浮越证应用清热化痰、泻火安神法。Dreaminess is a sleep disorder characterized by recurrent scary,restless or anxious dream experiences during the sleep cycle.Based on the theory of “liver storing ethereal soul”,this paper analyzes the etiology,pathogenesis and the thought of syndrome differentiation and treatment of comprehensive therapy of dreaminess.It is believed in traditional Chinese medicine that dreaminess is caused by liver suffering from pathogen,liver failing to store the ethereal soul,the ethereal soul failing to keep to its abode,frenetic stirring of the liver ethereal soul.Comprehensive therapy of dreaminess containing emotional therapy and pharmacotherapy has been used based on the theory of “liver storing ethereal soul” in order to treat both the body and mind.The emotional therapy includes the method of shifting spirit to regulate qi,the method of empathy from the idea,the method of restriction among emotions,and treating fright by calming.The pharmacotherapy focuses on treating liver.Methods of nourishing water to moisten wood and enriching yin to tranquilize the ethereal soul should be used in case of water failing to nourish wood and frenetic stirring of the liver ethereal soul syndrome.Methods of tranquilizing mind and emotion,consolidating the liver ethereal soul should be used in case of mental deficiency with timidity and liver failing to store ethereal soul syndrome.Methods of clearing heat and resolving phlegm,purging fire and tranquilizing mind should be used in case of internal disturbing of phlegm-fire and floating of the liver ethereal soul.
