摘 要:目的了解儿童青少年体脂分布与肺功能的关联性。方法纳入2019年1月至2019年12月在北京积水潭医院儿科就诊的单纯性肥胖儿童。采用生物电阻抗法检测患儿的身体成分,包括体重指数(BMI)、体脂百分比(FMP)、内脏脂肪面积(VFA)、躯干-全身脂肪比(TrTFR)、躯干-四肢脂肪比(TrLFR)、四肢-全身脂肪比(LTFR)、内脏-全身脂肪比(VTFR)。进行肺通气功能测定。偏相关分析体脂分布指标与肺功能的关系。结果共纳入107例患儿。男性58例(54.2%),女性49例(45.8%)。在肥胖男童和肥胖女童中,BMI与FVC%、FEV_(1)%呈负相关(P<0.05),FMP与FVC%、FEV_(1)%、PEF%负相关(P<0.05),VFA与FVC%、FEV_(1)%、PEF%、FEF50%、FEF75%呈负相关(P<0.05)。在肥胖男童中,TrLFR与FVC%、FEV_(1)%、PEF%、FEF50%、FEF75%呈负相关(r值为-0.216、-0.162、-0.271、-0.092、-0.132,P<0.05)。在肥胖女童中,TrLFR与FVC%、FEV_(1)%、PEF%、FEF50%、FEF75%呈负相关(r值为-0.182、-0.281、-0.209、-0.076、-0.168,P<0.05)。结论VFA、TrLFR对于肺功能的影响更多,对肥胖儿童,需关注脂肪分布对肺功能的影响。Objective To investigate the correlation between body fat distribution and lung function in children and adolescents.Methods From January 2019 to December 2019,children with simple obesity were enrolled in the Pediatric Department of Beijing Jishuitan Hospital.Body fat distribution was assessed by bioelectric impedance analysis including body mass index(BMI),fat mass percentage(FMP),visceral fat area(VFA),trunk to total fat ratio(TrTFR),trunk to limb fat ratio(TrLFR),limb to total fat ratio(LTFR),and viscera to total fat ratio(VTFR).The pulmonary ventilation function was measured.The relationship between body fat distribution indexes and lung function was analyzed by partial correlation analysis.Results A total of 107 children were included.There were 58 males(54.2%)and 49 females(45.8%).In obese boys and girls,BMI was negatively associated with FVC% and FEV_(1)%(P<0.05).FMP was negatively correlated with FVC%,FEV_(1)% and PEF%(P<0.05).VFA was negatively correlated with FVC,FEV_(1),PEF,FEF50 and FEF75(P<0.05).In obese boys,TrLFR was negatively correlated with FVC,FEV_(1),PEF,FEF50 and FEF75(r=-0.216,-0.162,-0.271,-0.092,-0.132;P<0.05).In obese girls,TrLFR was negatively correlated with FVC,FEV_(1),PEF,FEF50 and FEF75(r=-0.182,-0.281,-0.209,-0.076,-0.168;P<0.05).Conclusion VFA and TrLFR have more influence on lung function.In obese children,the influence of fat distribution on lung function should be paid more attention.
