摘 要:目的 分析大连市友谊医院2020年大肠埃希菌的临床分布以及耐药性,为临床治疗大肠埃希菌感染提供理论依据。方法 收集大连市友谊医院2020年1月2020年—12月大肠埃希菌共295株,进行体外细菌培养、鉴定和药敏实验。用WHONET5.6分析药敏情况,SPSS19.0 软件经χ;检验对尿液和非尿液标本检出菌株的耐药率进行分析。结果 295株大肠埃希菌主要来自尿液(181株);病区主要分布在泌尿外科(27.11%),其次肝胆外科(13.22%)和老年病科(9.83%);药敏结果显示大肠埃希菌对氨苄西林的耐药率最高(85.1%),而对呋喃妥因的耐药率最低(1.7%),其次为碳青霉烯类的美罗培南(3.7%),来自尿液标本的大肠埃希菌对环丙沙星和左氧氟沙星的耐药率为84.4%、77.2%,非尿液标本大肠埃希菌对这两种药物的耐药率分别为66.7%、63.6%,差异具有统计学意义(P<0.05)。结论 大肠埃希菌是临床最常见的病原菌,应严格控制处方权,以减缓耐药现象,提高临床抗感染质量。Objective This paper analyzes the clinical distribution and drug resistance of Escherichia coli(E.Coli) from January 2020 to December 2020 at Dalian Friendship Hospital with a view to providing theoretical basis for clinicians’ clinical diagnosis and medication of patients infected with Escherichia coli.Methods 295 strains of E.Coli in Dalian Friendship Hospital from January 2020 to December 2020 were collected for in vitro drug sensitivity test.WHONET 5.6 was used to analyze the drug sensitivity,and χ;tests were conducted to analyze the bacterial resistance rate in urine and non-urine samples.Results Clinically,295 E.Coli detected in different specimens were mainly distributed in urine(181).The main distribution area of E.Coli is urology(27.11%),hepatobiliary surgery(13.22%) and geriatrics department(9.83%).The drug resistance rate of E.Coli to Ampicillin was the highest(85.1%),while the drug resistance rate to furadantin was the lowest (1.7%),carbapenems meropenem was the second(3.7%),the drug resistance rates of E.Coli from urine samples to ciprofloxacin and levofloxacin were 84.4% and 77.2%,respectively,while those from non-urine samples were 66.7% and 63.6%,respectively,the difference was statistically significant(P<0.05).Conclusion E.Coli is the most common pathogenic bacteria in clinic,we should control the prescription power strictly to reduce the drug resistance of E.Coli,and improve the clinical anti-infection quality.
