宫安康联合黄体酮对稽留流产患者清宫术后宫腔粘连的预防作用

摘　　要：目的:探讨用宫安康(宫腔用交联透明质酸钠凝胶)联合黄体酮对行清宫术的稽留流产患者进行治疗对预防其术后发生宫腔粘连的效果。方法:选取2019年10月至2021年1月期间东莞三局医院收治的80例稽留流产患者作为研究对象。按照随机数表法将其分为常模组和干预组,每组各有患者40例。对两组患者均进行清宫术,术后对其均进行常规的预防性抗感染治疗,在此基础上用宫安康联合黄体酮对干预组患者进行治疗。比较两组患者术后腹痛持续的时间、阴道的出血量、阴道出血持续的时间、月经复潮的时间、性激素的水平及术后宫腔粘连的发生率。结果:干预组患者术后阴道的出血量少于常模组患者,其术后腹痛持续的时间、阴道出血持续的时间和月经复潮的时间均短于常模组患者,差异有统计学意义(P<0.05)。治疗后,干预组患者血清雌二醇的水平高于常模组患者,其血清促黄体生成激素和促卵泡生成激素的水平均低于常模组患者,差异有统计学意义(P<0.05)。干预组患者术后宫腔粘连的发生率为10.00%(4/40),常模组患者术后宫腔粘连的发生率为30.00%(12/40),二者相比差异有统计学意义(P<0.05)。结论:用宫安康联合黄体酮对行清宫术的稽留流产患者进行治疗的效果显著,能有效减少其术后阴道的出血量,缩短其术后恢复的时间,改善其性激素的水平,降低其术后宫腔粘连的发生率。Objective:To explore the effect of gong Ankang(intrauterine crosslinked sodium hyaluronate gel)combined with progesterone on the prevention of postoperative intrauterine adhesions in patients with delayed abortion undergoing uterine clearance operation.Methods:A total of 80 patients with delayed abortion admitted to Dongguan Third Bureau Hospital from October 2019 to January 2021 were selected as the study subjects.According to the random number table method,they were divided into the norm group and the intervention group,with 40 patients in each group.Patients in both groups were treated with uterine clearing,routine preventive anti-infection treatment after surgery,and patients in the intervention group were treated with gong Ankang combined with progesterone.The duration of postoperative abdominal pain,the amount of vaginal bleeding,the duration of vaginal bleeding,the time of menstrual revulsion,the level of sex hormones and the incidence of postoperative intrauterine adhesions were compared between the two groups.Results:The amount of postoperative vaginal bleeding in the intervention group was less than that in the norm group,and the duration of postoperative abdominal pain,vaginal bleeding and menstrual remenstruation were shorter than that in the norm group,with statistical significance(P<0.05).After treatment,the level of serum estradiol in the intervention group was higher than that in the normal group,and the levels of serum luteinizing hormone(LUteinizing hormone)and follicle-stimulating hormone(follicle-stimulating hormone)were lower than that in the normal group,with statistical significance(P<0.05).The incidence of postoperative intrauterine adhesions was 10.00%(4/40)in the intervention group and 30.00%(12/40)in the normal group,with statistically significant difference(P<0.05).Conclusion:The treatment effect of gong Ankang combined with progesterone on patients with uterine clearance abortion is significant,which can effectively reduce the amount of postoperative vaginal bleeding,shorten the time
