能谱CT联合常规CT检查、血清CA125检测在鉴别诊断卵巢囊性病变性质中的应用价值

摘　　要：目的:探讨能谱CT联合常规CT检查、血清糖类抗原125(CA125)检测在鉴别诊断良恶性卵巢囊性病变中的应用价值。方法:选择重庆市大足区人民医院收治的88例卵巢囊性病变患者作为研究对象。按照病理检查结果将其分为良性组和恶性组。对两组患者均进行常规CT检查、能谱CT检查及血清CA125检测,比较其检查/检测结果,并统计用能谱CT联合常规CT检查、血清CA125检测诊断良恶性卵巢囊性病变的效能。结果:恶性组患者血清CA125的水平高于良性组患者,P<0.05。对两组患者进行常规CT检查的结果显示,其中良性组患者中被诊断患有良性卵巢囊性病变的患者有50例,恶性组患者中被诊断患有恶性卵巢囊性病变的患者有21例。对两组患者进行能谱CT检查的结果显示,恶性组患者的CT值、碘浓度、水浓度和斜率在动脉期和静脉期均高于良性组患者,P<0.05。与用血清CA125检测、常规CT检查或能谱CT检查相比,用能谱CT联合常规CT检查、血清CA125检测诊断良恶性卵巢囊性病变的灵敏度、特异度和准确率均更高,P<0.05。结果:能谱CT联合常规CT检查、血清CA125检测在鉴别诊断良恶性卵巢囊性病变中具有较高的应用价值。Objective:To explore the value of energy spectrum CT combined with routine CT examination and serum carbohydrate antigen 125(CA125)detection in the differential diagnosis of benign and malignant ovarian cystic lesions.Methods:a total of 88 patients with ovarian cystic disease admitted to Dazu District People’s Hospital in Chongqing were selected as the research object.According to pathological examination results,they were divided into benign group and malignant group.Conventional CT examination,energy spectrum CT examination and serum CA125 detection were performed in the two groups,and the examination/detection results were compared,and the efficacy of energy spectrum CT combined with conventional CT examination and serum CA125 detection in the diagnosis of benign and malignant ovarian cystic lesions was analyzed.Results:The level of serum CA125 in malignant group was higher than that in benign group,P<0.05.Conventional CT examination of the two groups showed that 50 patients in the benign group were diagnosed with benign ovarian cystic lesions and 21 patients in the malignant group were diagnosed with malignant ovarian cystic lesions.The results of energy spectrum CT showed that the CT value,iodine concentration,water concentration and slope in the malignant group were higher than those in the benign group in the arterial phase and venous phase,P<0.05.Compared with serum CA125,conventional CT or ENERGY spectrum CT,the sensitivity,specificity and accuracy of energy spectrum CT combined with conventional CT and serum CA125 in the diagnosis of benign and malignant ovarian cystic lesions were higher,P<0.05.Results:Energy spectrum CT combined with routine CT examination and serum CA125 detection have high application value in differential diagnosis of benign and malignant ovarian cystic lesions.
