摘 要:目的基于广西永福长寿队列,探索血清P物质(substance P,SP)和β内啡肽(beta endorphin,β-EP)水平与长寿(年龄>85岁)和健康老化分数的关联。方法选取广西永福长寿之乡2016年的横断面调查人群251例,其中长寿老人127例(年龄86~111岁,长寿组),非长寿人群124例(非长寿对照组)。通过入户调查,收集老年健康相关人口学指标,测定血脂、血压等常规指标,并对长寿老人分别采用简易精神状态检查量表和日常生活活动量表评估认知和行为功能,进一步加权计算健康老化分数。基于Millipore Luminex 200技术检测血清SP和β-EP水平。对比血清SP和β-EP在长寿组和非长寿对照组的差异;基于Pearson相关分析研究长寿人群中血清SP和β-EP与年龄、血脂、血压等指标是否具有特殊相关性;进一步在长寿人群按健康老化分数进行综合功能分层,对比SP和β-EP在各层间的差异,绘制受试者操作特征曲线以评估组合SP和β-EP对长寿人群老化功能区分的价值。结果长寿组血清SP、β-EP水平和非长寿对照组无显著性差异;但长寿组内按健康老化分数分层后的组间比较发现,随着健康老化功能下降,血清SP和β-EP水平均存在显著下降趋势(均P<0.001);SP水平和总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、舒张压显著负相关,β-EP水平和TC、LDL-C显著负相关(均P<0.05);联合血清SP、β-EP(模型1)判断长寿人群健康老化功能的效果优于单独使用年龄(模型2),曲线下面积分别为0.769(95%CI:0.629~0.908,P=0.001)和0.527(95%CI:0.370~0.684,P=0.733)。结论血清SP、β-EP在长寿人群和非长寿人群差异无显著性,但与长寿老人内在能力显著相关,是健康老化分数的潜在保护因素,二者联合应用具有较好的预测老年人功能状态的价值。Objective Based on the Guangxi Yongfu Longevity Cohort,to explore the association between serum substance P(SP),beta endorphin(β-EP)levels and longevity(age>85 years),healthy aging score.Method Based on the crosssectional survey population(n=251)in Yongfu longevity county,Guangxi in 2016,there were 127 longevity elderly people(aged 86-111 years old,the longevity group)and 124 non-longevity people(the control group).By using household surveys,health-related demographic indicators were collected,routine indicators such as blood lipids and blood pressure were measured,and the Mini-Mental State Examination(MMSE)scale and the Activities of Daily Living(ADL)scale were used to evaluate the cognitive and physical functions,and the healthy aging score was further weighted to calculate.Serum substance P(SP)and beta endorphin(β-EP)levels were detected based on Millipore Luminex 200 technology.T test orχ2 teat was used to compare the differences of serum SP andβ-EP between long-lived and non-long-lived groups.Pearson correlation analysis was used to detect the correlation between serum SP andβ-EP and age,blood lipids,blood pressure and other indicators in long-lived people.The healthy aging score was used for comprehensive functional stratification.The differences between SP andβ-EP in each stratum were compared,and the receiver operating characteristic(ROC)curve was drawn to assess the value of combining SP andβ-EP for functional decline status in long-lived subjects.Result There was no significant difference in serum SP andβ-EP levels between the longevity group and the control group.However,within the longevity group stratified by healthy aging scores,a trend was found that with the decline of healthy aging function,serum SP andβ-EP significantly decreased(all P<0.001).SP levels were significant negatively correlated with total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),and diastolic blood pressure.β-EP levels were significant negatively correlated with TC and LDL-C(all P<0.05).The effect of mode
