足内侧动脉皮瓣与腕横纹皮瓣修复手指缺损的疗效比较

摘　　要：目的比较足内侧动脉皮瓣与腕横纹皮瓣应用于手指创面缺损的临床疗效。方法选取2018年7月至2020年1月收治的手指缺损患者40例(男30例,女10例),随机分为A、B两组,每组20例,分别行足内动脉皮瓣术和腕横纹肌皮瓣术。采用中华医学会手外科学会功能评定试用标准评级:外观评分,感觉评级,TAM评级。并对其进行统计学分析。结果术后患者均获随访,随访时间6~18个月,术后外形评分,足内侧动脉皮瓣组与腕横纹皮瓣组差异无统计学意义(P>0.05)。感觉评级,足内侧动脉皮瓣评分优于腕横纹皮瓣组(P<0.05)。TAM评分,足内侧动脉皮瓣组与腕横纹皮瓣组差异无统计学意义(P>0.05)。结论修复手指缺损患者应用足内侧动脉皮瓣较腕横纹皮瓣具有感觉优势,但两者在外观和功能恢复上无明显差别。Objective To compare the clinical effects of repairing finger defects with medial foot artery flap and wrist transverse flap.Methods From July 2018 to January 2020,40 patients(30 males,10 females)with finger defects were randomly divided into two groups:group A and group B,with 20 cases in each group,which were treated with medial plantar artery flap and wrist crease flap re⁃spectively.The functional evaluation trial standard of Chinese Medical Association Hand Surgery Society was used to evaluate the grade:appearance rating,sensory rating and TAM rating.The results were statistically analyzed.Results All patients were followed up for 6 to 18 months.There was no significant difference in postoperative appearance score between the medial foot artery flap and the wrist transverse flap group(P>0.05).In sensory rating,the medial foot artery flap was superior to the wrist transverse flap group(P<0.05).TAM score was not significant between the medial foot artery flap group and the wrist transverse flap group(P>0.05).Conclu-sion The medial foot artery flap has sensory advantage than the wrist transverse flap in repairing finger defect,but there was no obvious difference in appearance and function recovery.
