摘 要:目的:探讨百普力对胃癌术后患者营养状况和免疫功能影响。方法:选择本院2016年1月—2021年2月胃癌术后患者90例,按照随机数字表法分为两组,各45例。两组患者均给予胃肠道减压、抗感染、解痉、止痛等常规治疗。在此基础上,对照组患者给予肠外营养支持治疗,于术后第1日经中心静脉输注三大营养素,包括脂肪乳、葡萄糖和氨基酸;研究组患者给予百普力肠内营养支持治疗,术后12~24 h经鼻肠营养管输入300~500 ml的5%葡萄糖,于术后24~48 h输入500 ml百普力,于术后3~7 d输入500~1000 ml/d百普力。两组疗程均为7 d。比较两组患者胃肠功能恢复和术后并发症情况,术前1 d和术后7 d营养状况[白蛋白(ALB)、前白蛋白(PA)和总蛋白(TP)]和免疫功能变化。结果:研究组胃癌术后患者首次排便时间、首次排气时间和住院时间短于对照组(P<0.05)。研究组胃癌术后患者并发症(8.89%)少于对照组(26.67%)(P<0.05)。两组术前1 d患者ALB、PA和TP水平比较无明显差异(P>0.05);两组术后7 d患者ALB、PA和TP水平高于术前1 d(P<0.05);研究组术后7 d患者ALB、PA和TP水平高于对照组(P<0.05)。两组术前1 d患者CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)比较无明显差异(P>0.05);两组术后7 d患者CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)高于术前1 d(P<0.05);研究组术后7 d患者CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)高于对照组(P<0.05)。结论:百普力可改善胃癌术后患者营养状况和免疫功能,且可减少并发症发生,值得临床研究。Objective:To investigate the effects of peptisorb liquid on nutritional status and immune function of postoperative patients with gastric cancer.Methods:According to the random table method,a total of 90 patients with postoperative gastric cancer in our hospital from January 2016 to February 2021 were selected and divided into two groups,with 45 cases in each group.Patients in both groups were given conventional treatments,such as gastrointestinal decompression,anti-infection,crystallization,pain relief,and other routine treatment.On this basis,the control group was given parenteral nutrition support therapy,and three major nutrients,including fat emulsion,glucose,and amino acids,were injected through a central vein on the first postoperative day.Patients in the study group were given peptisorb liquid enteral nutrition support treatment and received 300~500 ml of 5% glucose through the nasal enteral nutrition tube at 12~24 hours after surgery,and 500 ml of peptisorb liquid at 24 to 48 hours after surgery.3~7 days after the operation,the dose of peptisorb liquid increased to 500~1000 ml/d.The treatment course of both groups was seven days.Gastrointestinal function recovery and postoperative complications,nutritional status[albumin(ALB),prealbumin(PA),and total protein(TP)],and immune function changes were compared between both groups.Results:The time of first defecation,fist exhaust,and hospital stay in the study group were shorter than those in the control group(P<0.05).The postoperative complications of gastric cancer in the study group(8.89%)were less than those in the control group(26.67%)(P<0.05).There was no significant difference in the levels of ALB,PA,and TP in patients with gastric cancer one day before surgery between the two groups(P>0.05).The levels of ALB,PA,and TP in both groups on day 7 after surgery were higher than those on 1 day before surgery(P<0.05).The nutritional status levels in the study group were significantly higher than those in the control group on day 7 after surgery(P<0.05).There we
