摘　　要：目的探讨老年阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清骨膜蛋白(Periostin)、能量平衡相关蛋白(Adropin)水平与睡眠呼吸参数、血管内皮功能、颈动脉内膜中层厚度(IMT)的相关性。方法选取2017年5月至2021年5月张家口市第一医院收治的老年OSAHS患者113例,根据OSAHS严重程度分成轻度组34例、中度组49例、重度组30例,另择同期体检的老年健康志愿者66名为对照组。比较各组血清Periostin、Adropin水平,并比较轻、中、重度组患者夜间最长呼吸暂停时间(LAT)、呼吸暂停低通气指数(AHI)、夜间最低经皮氧饱和度(LSpO_(2))、夜间平均SpO_(2)(MSpO_(2))、夜间SpO_(2)低于90%时间占总睡眠时间百分比(SLT90%)、觉醒指数、一氧化氮(NO)水平、内皮素-1(ET-1)水平及颈动脉IMT。分析OSAHS患者血清Periostin、Adropin水平与睡眠呼吸参数、血管内皮功能指标、颈动脉IMT的相关性。结果重度组患者血清Periostin水平>中度组>轻度组>对照组(均P<0.05),重度组患者血清Adropin水平<中度组<轻度组<对照组(均P<0.05)。重度组患者LAT、AHI、SLT90%、觉醒指数>中度组>轻度组(均P<0.05),重度组患者LSpO_(2)、MSpO_(2)<中度组<轻度组(均P<0.05)。重度组患者血清ET-1水平、颈动脉IMT>中度组>轻度组(均P<0.05),重度组患者血清NO水平<中度组<轻度组(均P<0.05)。OSAHS患者血清Periostin水平与LAT、AHI、SLT90%、觉醒指数、ET-1水平、颈动脉IMT均呈正相关(均P<0.05),与LSpO_(2)、MSpO_(2)、NO水平均呈负相关(均P<0.05);血清Adropin水平与LAT、AHI、SLT90%、觉醒指数、ET-1水平、颈动脉IMT均呈负相关(均P<0.05),与LSpO_(2)、MSpO_(2)、NO水平均呈正相关(均P<0.05)。结论老年OSAHS患者血清Periostin水平升高,而Adropin水平下降,并随着病情程度加重而更明显,且与睡眠呼吸参数、血管内皮功能紊乱及颈动脉IMT有关。检测血清Periostin、Adropin水平有助于评估老年OSAHS患者的病情�Objective To investigate serum periostin protein(Periostin)and energy balance related protein(Adropin)levels and their relationship with relevant parameters in patients with obstructive sleep apnea-hypopnea syndrome(OSAHS).Methods A total of 113 elderly patients with OSAHS treated in Zhangjiakou First Hospital from May 2017 to may 2021,and 66 healthy subjects(control group)were enrolled in the study.According to the severity of OSAHS,they were divided into mild group(n=34),moderate group(n=49)and severe group(n=30).The serum Periostin and Adropin levels were measured in all subjects.The longest apnea time(LAT),apnea/hypopnea index(AHI),lowest SpO_(2)(LSpO_(2)),nocturnal mean SpO_(2)(MSpO_(2)),percentage of time when oxygen saturation lower than 90%(SLT90%),arousal index,nitric oxide(NO)levels,endothelin-1(ET-1)levels and carotid IMT were compared among mild,moderate and severe groups.The correlation of Periostin and Adropin levels with sleep breathing parameters,vascular endothelial function indexes and carotid intima-media thickness(IMT)in patients with OSAHS were analyzed.Results The serum Periostin levels were decreased in order of severe group,moderate group,mild group and control group(all P<0.05),while the serum Adropin levels increased in order of severe group,moderate group,mild group and control group(all P<0.05).The LAT,AHI,SLT90%,arousal index were decreased in order of disease severity in OSAHS patients(all P<0.05),while LSpO_(2),MSpO_(2) were decreased in order of disease severity(all P<0.05).The ET-1,carotid IMT were increased in order of disease severity(all P<0.05),and NO level was decreased in order of disease severity(all P<0.05).Serum Periostin level in OSAHS patients was positively correlated with LAT,AHI,SLT90%,arousal index,ET-1,carotid IMT(all P<0.05),and negatively correlated with LSpO_(2),MSpO_(2),NO(all P<0.05).Serum Adropin level was negatively correlated with LAT,AHI,SLT90%,arousal index,ET-1,carotid IMT(all P<0.05),and positively correlated with LSpO_(2),MSpO_(2),NO(all P<0.05).Conclu
