摘  要:目的:使用新型智能穿刺手术导航设备引导不同部位穿刺活检及消融手术,探讨其广泛的潜在临床应用价值。方法:选取2021年8月—10月,在西安交通大学第一附属医院行穿刺手术导航IQQA®-Guide系统辅助CT引导进行微创诊疗患者23例,其中穿刺活检11例,射频消融6例,放射性125I粒子植入6例。将患者术前术中影像数据资料及解剖数据资料导入IQQA®-Guide系统,融合生成电子组织数据地图并手动选择靶目标点。将电磁跟踪适配器连接于穿刺针,穿刺过程中参考系统显示的三维针道路径图和电子组织数据地图实时监测引导完成进针操作,到达目标点后进行CT扫描以确认穿刺针位置,判断是否需要调整,如果穿刺针位置满意,则进行穿刺活检取材、射频消融或粒子植入等诊疗操作。结果:23例患者中有1例粒子植入患者因电磁定位片旁金属异物干扰,导致空间配准出现错误,未能进行导航穿刺,其余22例患者均在IQQA®-Guide系统引导下顺利完成穿刺活检及消融治疗操作,未出现严重并发症。结论:智能穿刺手术导航IQQA®-Guide系统可辅助多种胸腹部CT引导下微创术式,其安全、高效、智能、实时动态监测等优点有望成为一项具有广阔应用前景和临床推广价值的影像引导技术。Objective To explore the potential clinical value of using new intelligent puncture surgical navigation equipment to guide puncture biopsy and ablation at different parts.Methods 23 patients receiving the minimally invasive diagnosis and treatment assisted by IQQA®-Guide system of puncture operation navigation and guided by CT in our hospital from August to October 2021 are collected,including 11 cases of puncture biopsy,6 cases of radiofrequency ablation,and 6 cases of implantation of radioactive 125 I particles.The preoperative and intraoperative imaging data and anatomical data of patients are imported into the IQQA®-Guide system to generate the electronic tissue data map and target points are manually selected.The electromagnetic tracking adapter is connected to the puncture needle.During the puncture,the 3D needle path diagram and electronic tissue data map displayed in the reference system are used for real-time monitoring and guidance of the puncture operation.After the needle reaches the target point,CT scan is performed to confirm the position of the puncture needle and determine whether adjustment is required.If the position of the puncture needle is satisfying,the sampling of puncture and biopsy,radiofrequency ablation,particle implantation,or other diagnosis and treatment operation are performed.Results Among the 23 patients,1 patient with particle implantation fails to perform the navigation puncture due to spatial registration errors caused by the interference of metal foreign bodies next to the electromagnetic locating plate.The remaining 22 patients smoothly complete puncture biopsy and ablation treatment operation under the guidance of the IQQA®-Guide System without serious complications.Conclusion IQQA®-Guide system of the intelligent puncture navigation can assist various minimally invasive operations guided by many kinds of thoracic and abdominal CT.With safety,high efficiency,intelligence,real-time dynamic monitoring and other advantages,it is expected to become an image guidance technolo
