摘 要:目的 探讨不同呼吸道感染患儿血清25-羟维生素D[25(OH)D]和免疫指标的差异,分析不同血清25(OH)D营养状态与儿童钙磷代谢、免疫指标的差异。方法 回顾性分析2019年1月至2020年5月延安大学附属医院收治的681例呼吸道感染儿童的临床资料,将其分为A组(上呼吸道感染,175例)和B组(下呼吸道感染,506例)。另选取100例同期健康体检儿童作为对照组。分析3组血清25(OH)D、免疫指标[包括免疫球蛋白(Ig)A、IgG、IgM、CD4^(+)和CD8^(+)]及钙磷代谢情况。结果 3组男女性别构成比比较,差异无统计学意义(P>0.05),3组不同年龄和血清25(OH)D营养状态构成比比较,差异均有统计学意义(P<0.05)。3组血清25(OH)D、血清钙、血清磷、IgG、IgA、IgM水平和CD4^(+)、CD8^(+)比较,差异均有统计学意义(P<0.05)。不同维生素D营养状态的呼吸道感染儿童钙磷代谢和免疫指标总体顺序由高到低依次为适宜、不足、缺乏、严重缺乏,差异均有统计学意义(P<0.05)。结论 血清25(OH)D水平降低可增加儿童呼吸道感染风险,不同血清25(OH)D营养状态的呼吸道感染儿童钙磷代谢和免疫指标差异明显。Objective To explore the differences of serum 25-hydroxyvitamin D[25(OH)D] and immune indexes in children with different respiratory tract infection,and to analyze the differences of different serum 25(OH)D nutritional status,calcium and phosphorus metabolism and immune indexes in children.Methods The clinical data of 681 children with respiratory tract infection admitted to Yan′an University Affiliated Hospital from January 2019 to May 2020 were retrospectively analyzed,and they were divided into group A(upper respiratory tract infection,175 cases) and group B(lower respiratory tract infection,506 cases).Another 100 healthy children were selected as the control group.Serum 25(OH)D,immune indexes,including immunoglobulin(Ig)A,IgG,IgM,CD4^(+) and CD8^(+),and calcium and phosphorus metabolism in the three groups were analyzed.Results There were no statistically significant differences in gender composition among the three groups(P>0.05),and there were statistically significant differences in proportion of age and serum 25(OH)D nutritional status among the three groups(P<0.05).There were statistically significant differences in levels of serum 25(OH)D,serum calcium,serum phosphorus,IgG,IgA,IgM,as well as CD4^(+) and CD8^(+) among the three groups(P<0.05).The overall order of calcium and phosphorus metabolism and immune indexes in children with respiratory tract infection with different vitamin D nutritional status were appropriate,insufficient,deficient and severely deficient,with statistical significance(P<0.05).Conclusion The decrease of serum 25(OH)D level could increase the risk of respiratory tract infection in children.Calcium and phosphorus metabolism and immune indexes of children with different serum 25(OH)D nutritional status were significantly different.
