陕西地区正常中年人全膝关节置换相关长度参数的数字影像测量与分析

摘　　要：目的:对陕西地区正常中年人双膝关节数字影像全膝关节置换(TKA)相关长度参数进行测量与分析,以期获得对假体设计和临床应用有参考意义的研究结果。方法:根据本研究纳入与排除标准,回顾性选取我院正常中年人双膝关节正侧位X线检查数字影像学资料91例(182膝),其中女性50例(100膝),男性41例(82膝),平均年龄为(50.29±5.51)岁。对所得样本用Photoshop CC64软件标识并测量相关指标:股骨内外髁横径、股骨前髁厚度、胫骨平台横径、胫骨平台前后径、DPF距离、髌骨横径、髌骨上下径、髌骨厚度。分别对性别间及左右侧数据进行统计学分析。结果:股骨内外髁横径、胫骨平台横径、胫骨平台前后径、髌骨横径、髌骨上下径、髌骨厚度在性别间相比,差异均具有显著的统计学意义(P<0.01),而股骨前髁厚度、DPF的性别间差异及所有指标的左右侧差异均无明显统计学意义(P>0.05)。结论:通过对正常中年人双膝关节数字影像资料进行测量,并与全膝关节置换相关长度参数进行分析,所得数据可作为正常中年人双膝关节数据的参考,对假体设计、术前规划、术中操作及术后评价均具有重要的理论意义与实践价值。Objective To measure and analyze the related length parameters of total knee arthroplasty(TKA)of digital imaging for normal middle-aged people in Shaanxi area,in order to obtain the research results of reference significance for prosthesis design and clinical application.Methods According to the inclusion and exclusion criteria of this study,91 cases(182 knees)of digital imaging data of anteropositive and lateral X-ray examination of double knee joints of normal middle-aged people in our hospital were retrospectively selected,including 50 cases(100 knees)of females and 41 cases(82 knees)of males,with an average age of(50.29±5.51)years old.The samples obtained were marked with Photoshop CC64 software and related indicators were measured,including transverse diameter of internal and external condyle of femur,thickness of femoral anterior condyle,transverse diameter of tibial plateau,anterior and posterior diameter of tibial plateau,DPF distance,transverse diameter of patella,upper and lower diameter of patella,and thickness of patella.The data were statistically analyzed between two genders and left and right sides.Results There were statistically significant differences in the transverse diameter of internal and external condyle of femur,transverse diameter of tibial plateau,anteroposterior diameter of the tibial plateau,transverse diameter of patella,suprainferior diameter of patella,and thickness of patella between genders(P<0.01),while there are no statistically significant differences in the thickness of the preartis of femur,DPF between genders,and all indexes between the left and right sides(P>0.05).Conclusion In this study,by measuring digital image data of double knee joints of normal middle-aged people and analyzing the related length parameter of total knee replacement,the data obtained can be used as the reference of data of double knee joints of normal middle-aged people.The prosthesis design,preoperative planning,intraoperative operation and postoperative evaluation have important theoretical signifi
