益肾暖宫方联合针灸对多次IVF-ET肾阴虚患者卵巢储备功能、子宫内膜厚度及妊娠结局的影响

摘　　要：目的探讨益肾暖宫方联合针灸对多次体外受精-胚胎移植(In vitro fertilization-embryo transfer,IVF-ET)肾阴虚患者临床疗效、卵巢储备功能、子宫内膜厚度及妊娠结局的影响。方法选取2017年5月—2019年5月期间在贵阳市妇幼保健院行多次IVF-ET患者98例,按照随机数字表法分为对照组和研究组,每组各49例。对照组给予常规+针灸治疗,研究组给予常规+针灸+益肾暖宫方治疗。观察比较两组患者临床疗效,卵巢储备功能[基础窦卵泡数与性激素,性激素包括促黄体生成素(Luteinizing hormone,LH)、促卵泡刺激素(Follicle stimulating hormone,FSH)、雌二醇(Estradiol,E_(2))],子宫内膜厚度,子宫动脉血流动力学状态[子宫动脉阻力指数(Resistance index of uterine artery,RI)、子宫动脉搏动指数(Plusatility Index,PI)],妊娠结局及不良反应。结果治疗后研究组临床总有效率为91.84%(45/49),高于对照组的75.51%(37/49),两组比较,差异有统计学意义(P<0.05)。治疗后两组患者基础窦卵泡数均较治疗前升高,LH、FSH、E_(2)水平均较治疗前降低,差异有统计学意义(P<0.05);且研究组基础窦卵泡数较对照组升高,LH、FSH、E_(2)水平均较对照组降低,差异有统计学意义(P<0.05)。治疗后两组患者子宫内膜厚度较治疗前增厚,RI、PI指标较治疗前降低,差异有统计学意义(P<0.05);且研究组子宫内膜厚度较对照组增厚,RI、PI指标较对照组降低,差异有统计学意义(P<0.05)。治疗后研究组获卵数、临床妊娠率、优质胚胎率均较对照组升高,无可移植胚胎率较对照组降低,差异有统计学差异(P<0.05)。治疗期间,两组患者均无与药物相关的不良反应发生。结论益肾暖宫方联合针灸对多次IVF-ET肾阴虚患者具有较好的治疗效果,可改善卵巢储备功能,提高子宫内膜厚度,增加促排卵效果,从而进一步提高临床妊娠率。Objective To investigate the clinical efficacy,ovarian reserve,endometrial thickness and pregnancy outcome of Yishen Nuangong Decoction combined with acupuncture on multiple in vitro fertilization-embryo transfer(IVF-ET)patients with kidney-yin deficiency.Methods A total of 98 patients who underwent IVF-ET in Guiyang Maternal and Child Health Hospital from May 2017 to May 2019 were selected and divided into a control group and a study group according to the random number table method,with 49 cases in each group.The control group was given routine+acupuncture treatment,and the study group was given routine+acupuncture+Yishen Nuangong Decoction.The clinical curative effect,ovarian reserve function[based sinus follicle number,sex hormone,luteinizing hormone(LH),follicle-stimulatinghormone(FSH)and estradiol(E_(2))],endometrial thickness and uterine artery blood flow mechanics state[uterine artery resistance index(RI),uterine artery pulsation index(PI)],pregnancy outcome and adverse reactions of two groups were compared.Results After treatment,the total clinical effective rate in the study group was 91.84%(45/49),which was higher than 75.51%(37/49)in the control group,and the difference was statistically significant(P<0.05).After treatment,the number of basal antral follicles in the two groups were increased compared with those before treatment,and the levels of LH,FSH and E_(2)were lower than those before treatment,and the difference was statistically significant(P<0.05).Compared with the control group,the levels of LH,FSH and E_(2)were lower,and the difference was statistically significant(P<0.05).After treatment,the endometrial thickness of the two groups of patients was thicker than before treatment,and the RI and PI indexes were lower than those before treatment,and the difference was statistically significant(P<0.05).The PI index was lower than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the number of oocytes retrieved,the clinical pregnancy rate,and the hig
